Objective of Investigation
The fundamental objective of investigation is
the prevention of further accidents/incidents.

Definition - Accident
An occurrence associated with the operation of an
ACFT which takes place from the time when a person
boards the ACFT until disembarking in which :

a) person is fatally or seriously injured

b) ACFT sustains damage or structural failure

c) ACFT is missing or is completely inaccessible.

ACFT Proximity - AIRPROX

Is Classified as follows
a) RISK OF COLLISION
a serious risk of collision has existed.
b) SAFETY NOT ASSURED
safety may have been compromised.
a) NO RISK OF COLLISION
no risk of collision has existed
d) RISK NOT DETERMINED
insufficient or conflicting information to
determine the risk involved.

Air Traffic Incident
When an ACFT involved in an incident has a
destination outside the area of responsibility of the
ATS unit where the incident occurred,
the ATS unit at the destination aerodrome shall be
notified and requested to obtain the pilot’s report as
follows :

a) Type of incident [AIRPROX, Procedure or Facility]


b) Call Sign
c) Time and position of the incident
d) Brief details on the incident.

Analysis of ATS Incidents
The analysis of an incident should be considered
in relation to system operation and have regard to:

a) Procedures Separation procedures ?

b) Data and display Complete/Correct Information ?

c) Co-ordination Adequate and correct, fully applied ?

d) Communication Phraseology correct - Hear back ?

e) Equipment Performance of equipment adequate ?

f) Personnel Performance Fatigue, illness, negligence of ATCO ?

g) Task Environment Working environment sufficient ?

h) General operations Procedures known, system adequate ?
