PHYSICIAN ASSISTED SUICIDE

Instead of engaging in assisted suicide, physicians must aggressively


respond to the needs of patients at the end of life. Physicians:

1. Should not abandon a patient once it is determined that cure is


impossible.
2. Must respect patient autonomy.
3. Must provide good communication and emotional support.
4. Must provide appropriate comfort care and adequate pain
control.
What are they looking for?

 Communication and Empathy/Compassion – role-plays (in which you may break bad news,
have to find out particular information from a person or simply hold a conversation with a
stranger) and formal questions.

 Ethical Reasoning and Debates – prioritisation tasks, scenario tasks, questions on hot topics
and critical thinking scenarios (e.g. who gets the organ?)

 Professionalism and Probity – scenarios tasks (e.g. what would you do if….?) and
prioritisation tasks.

 Data Interpretation/Gathering – calculations, written tasks, scenario interviews, group tasks


and video tasks.

 Problem Solving – prioritisation and tasks that require critical thinking.

 Personal Qualities, Motivation, Work Experience, Medical Insight and Current Affairs –
formal questions.

 Teamwork – group tasks and formal questions.

Medical ethics
The 4 pillars of medical ethics:
1. Autonomy – respecting a patient’s ability to make their own decisions
2. Beneficence – a doctor should always act to benefit the patient
3. Non-maleficence – a doctor should not cause harm to the patient
4. Justice – reference to a consideration of the law and of the overall benefit to society

1. Organ donation- opt in/out


Currently there are roughly 5,000 people waiting on the transplant list, and it is estimated that
three people die every day while waiting for an organ transplant in the UK alone.

Organ transplant
 Age. Younger patients would generally live longer if given the liver transplant, meaning more
QALYs (Quality Adjusted Life Years) will be saved.
• Biological match. Are the patients biological matches for the liver, including HLA? Is the size
• of the liver appropriate for the size of the patient?
• Alcoholism status. Is there a risk of alcoholism? If so, will there eventually be a need for
another liver if they revert?
• Urgency. Consider the urgency of need for the liver transplant.
• Contribution to society/others. Do the patients have dependents? Are they gainfully
employed?

NHS Long-Term Plan focuses on and its key Ambitions


 Diseases prevention – decreasing obesity and smoking rates, reducing number of alcohol
intoxication-related A&E admittances as well as enrolling those at risk into Type II Diabetes
Prevention Programme have been all named as NHS goals for the next 10 years.

• Improved funding – the funding to the system over the next five years has been confirmed as
averaging 3.4% which is higher than the current 2%. This represents a major increase which
would help in enhancing the quality and accessibility of services. Moreover, better allocation
of resources to increase support in deprived areas has been suggested.

• Digital solutions – every patient will be able to access online GP appointments. This is
expected to reduce number of outpatient appointments and decrease pressure on the
system, while delivering care faster and of the same standard.

• Cancer Survival – improving early detection of cancer has been named as one of the goals
with an aim of getting a clear diagnosis within the first 28 days following referral to specialist
services by the GP.

• Understaffing – the plan sets out measures to increase number of nursing and medicine
undergraduate places, improve working conditions for current staff to reduce the number of
those leaving the NHS, improve professional development options.
