    AFFIDAVIT FOR CHANGE OF SIGNATURE

    I, KELVIN HARRY MATUNDA, adult male person of Post Office Box 6117,
    ARUSHA, a Christian and a citizen of the United Republic of Tanzania both
    hereby make oath and swears as follows:-

    1. That I am KELVIN HARRY MATUNDA and I am the holder of Account


    No. 0031027457 which is at the EXIM BANK Clock Tower Branch Arusha.

    2. That the said Account has been dormant for a long time and I want to
    reactivate the same.

    3. That due to that matter I have decided to change the signature that used
    to appear previously with a new signature which shall appear hereunder.

    I honestly sincerely and truly swear that all that which is stated hereinabove is
    true and correct to the best of my knowledge.
