CONSENT LETTER

TO WHOMSOEVER IT MAY CONCERN

I herewith state that I have given my consent for the photo shoot of my ward ________
studying in Grade/ Sec _________ being used by Envision High - The Future Plus School.

I further state that I have given my consent with my own wish & will without any prejudice.
All the statements made in this form are my own statements. I herewith give my consent to
Envision High - The Future Plus School to use these PHOTOHRAPHS AND VIDEO bites for
perpetuity in their advertisements (hoardings, print media, social sites… etc).

All the photographs & videos given by me to Envision High - The Future Plus School near
Reliance Mall, Basava Nagar Main Road Vignan Nagar, Doddanekundi Bangalore, Karnataka -
560 037 are my ward’s photographs and videos taken by professionals engaged, for which I
have the complete rights. I also herewith give my permission for the material to be used as it
is or recreate and use them in the production of the film for perpetuity by giving the image &
video credit.

This Consent letter has been issued by me upon the specific request of Envision High
Consultancy without any prejudice.
