    Dear Dr., Ragay,

    I’m writing this letter as a formal notification that I intend to not renew for the calendar year

    2023 for the position of Nutritionist – Dietitian (Contract of Services) at Bayawan District Hospital.

    I want to take this opportunity to thank you for your time and efforts in training and honing my

    skills and abilities. I would also want to thank my superiors and colleagues who supported me and

    guide me throughout my journey. I have greatly enjoyed working for the institution.

    My decision is final and I’m leaving for a career enhancing opportunity. I wish to leave

    Bayawan District Hospital on good terms with no animosity.

    I wish Bayawan District Hospital all its success in the future.
