Dear Sir,

Please accept this as my formal resignation letter from my position as Head Nurse to be
effective on May 8, 2020, for a change in career direction and for professional growth. I need
to accomplish my requirements in my masters and having tied up in the institution will be a
hindrance in pursuing it. When I finish my masters, I’m looking forward to work in academe.

Hereof, I would like to extend my appreciation and gratitude for the opportunity to learn
considerably and grow professionally during my fruitful tenure in First Cabuyao Hospital and
Medical Center Incorporation and it has been a rewarding experience and a pleasure to work
in this very reputable organization together with my very wonderful colleagues.

I am grateful for the support and I do regret any inconvenience my resignation will cause but
rest assured that I will support and ascertain a smooth transition and a seamless turnover as
possible.

I wish you all well and thank you for the opportunity to work for the furtherance of First
Cabuyao Hospital and Medical Center Incorporation.
