March 7, 2012 To: SILVERIO COPE, MD Chief of Hospital Armando D.

Cope Memorial Hospital Ruivibar Tayhi, Tabaco City Thru: GRACE ILLANA RN Chief Nurse Dear Sir/Madam: Good Day. I am writing to formally notify you that I am resigning from my position as a Staff Nurse effectively on March 12, 2012. I have decided and realize my personal goal to pursue my work as a community nurse under the umbrella of the DOH under the RNHEALS batch 3 programs. This was not an easy decision and took a lot of consideration but I believe it is an exciting step forward and I hope you will understand and support. Please be assumed that I will do all I can to assist in the smooth transfer of my responsibilities before Leaving. My experience working for the company is wonderful. You have welcomed me with open arms. Despite my shortcomings as an employee, you have remained patient and supportive in building me professionally and personally. For these, I am forever grateful to you and the Institution. I have always considered it an honor and privilege to be a part of the Health Team and it is my privileged to be part of this institution, for they had shown me their hospitality, warmth and guidance for the past months I spend my time in this Hospital. I have gained so much knowledge and skills which confide that I learned so much more than this hospital has to offer. Should you have any need for my service in the same area, I would of course consider returning once I have completed my RNHEALS Training Deployment under the Program of the DOH. In the meantime, I wish both you and the Armando D. Cope Memorial Hospital every good fortune and I would like to thank you once again for having me part of the team. I forward that someday I still have the opportunity to be part of this family. Thank you very much and may God always Guide our ways. Respectfully yours, EDWARD ANDREW E. GANDUL, RN ADCMH, Staff Nurse
