Thank you for being a Health Partners Medicare member. By partnering with Inovalon,
an independent company, we are excited to let you know about Personal Health Visits, a
new health and wellness program, which can be performed in person in the comfort of your own
home at a time that is convenient for you or via a Virtual Health Visit using the video function
on your smartphone, tablet, or computer.* We'Il be calling soon to give you more details and
look forward to speaking with you.

You have been selected to participate because we care about you and want to help you
improve your health. Participation is optional and there is no cost to you. Having a
Personal Health Visit allows us to learn more about your health and well-being.

A Personal Health Visit involves a nurse practitioner performing an assessment in person in the
comfort of your home at a time that's convenient for you or virtually. He or she will:
● Perform a physical exam
● Talk to you about your medical history and current needs
● Discuss your medications
● Provide you with time to ask questions and address your concerns
● Let you know of any changes to your health
● Educate you on health-related issues

The nurse practitioner will give you recommendations to ensure you get the best care. He
or she will also talk with you about important screenings or tests that you should have.

The nurse practitioners in the program are approved by Health Partners Medicare to
ensure the in-person visits are safe and profesional. However, if you feel more comfortable,
you're welcome to have a friend, family member or caregi ver present during your visit.

A Personal Health Visit does not replace any appointments with your doctor. As your
trusted health partner, Health Partners Medicare notified your doctor that you've been
invited to participate and described the program to them.

After your visit, you' Il get information about your health. You should keep this as a
reference. We'Il also send a report to your doctor. Health Partners Medicare and your
doctor will work closely together and use the report to maintain or improve your care and
coordinate additional resources that may benefit you.
