1 Choose a Primary Care Provider (PCP)

Log in to your online member account


to find an in-network doctor near you.
Check out the doctors on the site and
select one you’d like to see as your PCP.
As your main doctor, your PCP is who
you’ll see for annual wellness exams,
vaccinations, colds, fevers and ongoing
issues like allergies and asthma.

2 Schedule your annual wellness exam

Your annual wellness exam is included


in your plan — plus you will receive
500 My Health Pays® Rewards points
after your visit! An annual wellness
exam with your PCP can help you
catch problems early while they’re
easier to treat.1 It’s also a great time
to ask questions and talk about your
health goals.
We will deposit the additional 500
points into your My Health Pays
Rewards account once we confirm
that you have completed your annual
wellness exam. This could take
several weeks.

Nurse Advice Line. Your plan also offers
options for online care, urgent care, or
medical emergencies. Log in to your online
