Benefits Options

Medical
1. MEC/Basic Minimum Coverage covers mostly preventative care, copays apply. 100% coverage.
2. MVP/Buy-up plan – expanded coverage to include physician visits, ER visits, urgent care,
diagnostic services, hospital inpatient, prescriptions. Copays apply 100% coverage for
preventative care. Copays: $20 primary care, $40 specialist, $50 urgent care, $50 basic labs & x-
rays.
3. Anthem Health Plan, 3 Plan Options– expanded coverage to include physician visits, ER visits,
urgent care, specialists, diagnostic services, hospital inpatient, and prescriptions. Copays apply
100% coverage for preventative care.

Dental
1. Two plans offered (one in-network and one out-of-network). Pays
100% of preventative care. Basic Dental Services are covered at 100%.
2. Two plans offered (Value Plan and Network Access Plan). Pays 100%
of preventative care. Basic Dental Services are covered at 100% with
no deductible.

Vision
1. Includes annual exams and lenses for a copay, contact lenses each year and frames every
two years, up to $130/year.

Voluntary Benefits

Nationwide Pet Insurance - a benefit to help cover injuries, illness, and wellness care for
dogs, cats, birds and exotic pets.
401k Plan – Employee can choose from a variety of funds, Traditional and/or Roth
options are available. 401K is offered to all W2 employees after 6 months of
continuous employment.

Additional Benefits
Employee Assistance Program (EAP) – 24-hour support/ in person, via telephone, and
web portal for all employees to offer any assistance that you may need. This includes
up to five face-to-face sessions. This is free to all employees, regardless of your benefit
elections.
