• Step 1:  Visit the MedUnited Hospitals/Clinics/ Kiosk to learn more about the MedUnited Membership program.
• Step 2: Check your eligibility to become a member of the MedUnited Membership program. The program is open to
individuals and family members.
• Step 3: Fill out the application form for the MedUnited Membership program. The application form is available at the
hospital's front desk/ Clinic front desk/ Kiosk counter 
• Step 4: Submit the completed application form to the front desk at any MedUnited hospital or clinic. 
• Step 5: Pay the Membership price at the Front desk/ Kiosk to avail the Membership. Membership is approved from the
time of membership generation (No delay/ No waiting period) 
• Step 6:  Start enjoying the benefits of the MedUnited Membership program. These benefits include free outpatient
registration, discounted doctor consultations, free dietician and physiotherapist consultations, discounts on

•  Razole: Ultrasound, X Ray, ECG, PFT, Pathology and Doctor Consultation services available
• Thatipaka: Ultrasound, X Ray, ECG, PFT, Pathology and Doctor
• Malkipuram : Ultrasound, X Ray, ECG, TMT, Pathology and Doctor Consultation services available.

3. Is there an eligibility criteria for MedUnited Membership?
• Any individual, 18 years or above can sign up as a Primary user of the program. Individuals under 18 yrs of age can be added as dependent
users as part of a family plan only.
• There is no pre-existing health condition exclusion for eligibility 
• The plan prices remain the same for all eligible users.
• A valid government issued photo ID (Aadhar, PAN or Passport) is required for sign up and for availing the services at our centres.

5 . What payment options are available to sign up for MedUnited Membership plans?
Only one time payment is accepted to sign up for MedUnited Membership plans. No EMIs or Credit is available. The plan gets activated only
upon successful payment.

6. How long is my MedUnited Membership Card valid for?
This will depend on the tenure of the MedUnited Membership plan you have signed up for. Most plans are annual plans and are valid for 1 year
(365 days) from the date of purchase.
However, please read the benefits and validity details carefully prior to signing up for any plan as these may vary from plan to plan.

7. How long does it take for my MedUnited Membership plan to get activated after I sign up?
The plan gets activated instantly once the sign-up process is complete and payment is successfully made.

8. How long does it take for my plan benefits to take effect? 

These is no cooling off period for the MedUnited Membership Plan. The benefits are available immediately after a successful sign-up and
payment.
9 . How can I renew my MedUnited Membership Plan?
You will receive a reminder from MedUnited around 15 days prior to your current plan expiry. If you wish to renew, you will be able to do it
online through your account login or at any MedUnited Hospitals.

10. Can I cancel/withdraw my MedUnited Membership Plan?

It is not possible to cancel the MedUnited Membership Plan during the tenure of the plan

11. Will any price revisions affect my MedUnited Membership plan in the future?
Plan Charges - Revised plan charges will not affect existing users.
However, any new user added to the existing plan after a price revision will attract the revised, applicable charges per head.
Plan Benefits - Revised plan benefits will not affect existing benefits during the tenure of the plan. However, any changes in the prices
of diagnostic tests, profiles, packages and doctor consultations will be solely at the discretion of MedUnited.

12. Do I need to reach the centre before selected slot to avail the services?
Each service provided has a different turn around time. We Request you to reach the MedUnited 30 minutes prior to your slot.
We request your patience and support in case of delays in service due to unavoidable and emergency services.

13. Can I add my other relatives to the Membership ? 

As per the MedUnited membership plan primary member can add only Primary familly members to the plan.
