This is to certify that Mr. Frederick Viray, residing at Quezon City, was evaluated on February 13,2022
at CENTRALLE MEDICAL Clinic for physical and mental health clearance.

Diagnosis: t/c situational anxiety and situational depression secondary to work fatigue

Recommendation/Treatment:

The patient’s mental health was evaluated using interview and psychological screening tests PHQ-9 and
GAD, which measure the severity of depression and anxiety respectively. The patient got a total score of 25 for the
PHQ-9 and 21 for the GAD, which are translated as severe depression and severe anxiety respectively. The
preceding results supported the conclusion that the patient is not yet mentally prepared for work in correlation with
his clinical history and mental health findings.

Remarks:

The patient is NOT fit to work at this point in answering voice calls as this type of work triggers and
escalates his anxiety and depression. It is HIGHLY RECOMMENDED that the patient should be shifted or
transferred to a chat /email/non-voice type of work as this is tolerable for the current psychological and
mental health disposition of the patient
