    Re: Nurse for Clinic Management Services

    Dear Edson,

    Thank you for your letter of intent offering your services as an independent
    Company Nurse for our Clinic Management Services. We have reviewed your
    credentials and found the same more than satisfactory.

    We are very much interested in your services, which will be to cover for any
    absent Company Nurse in the project. In case we do avail of your services, you will
    be required to provide coverage of 12 hours shift in a day, with an hourly rate of
    Php 150.00. Should more hours be required in a given day, the legal overtime pay will
    apply.

    We will contact you as soon as any available work opens up on the free days
    you have provided us.

    Again, we thank you for your letter and hopefully we can work together in the
    future.
