We the fourth-year students of Bachelor of Science in Business Administration major in

Financial Management at the University of Eastern Pangasinan are currently working on
the Feasibility Study this 1st Semester A.Y 2022-2023 entitled, “YarnsToStitches” as a
partial fulfillment to the subject Feasibility Study.

In this regard, we would like to ask your permission to allow us to conduct a survey among
your students. The survey will last only about 5 – 10 minutes. Rest assured that the data
gathered will remain confidential and for academic purposes only.
