Good clinical practice is the core of the practicum exposure which is relevant to
specialization I want to pursue. The focus should be on a thorough assessment of
patients to provide the information necessary to determine differential diagnoses and
treatments. Critical thinking skills should be developed during these exposure which
LPU has made this possible in concert with practical skills, giving an understanding on
when their practical skills are necessary or when careful monitoring is sufficient
moreover to the welfare of Filipino people in terms of achieving optimal health and
wellness.

My specific goal I want to attain during my the whole 40 hours of relevant


exposure which I personally chose to one government hospital in Batangas is ensuring
optimal patient care through objective, systematic monitoring, established standards,
and criteria-based evaluations. Also added to my list is to care for patients with acute
and chronic illnesses; facilitating discharge planning; and offering patient education;
illness prevention services, and health maintenance care.

My client during my hospital exposure was a senior citizen which is close to my


heart since I believe that every elderly needs a safe and quality nursing care to all
health care professionals specially those who are indigent individual. He is a 67 year
old, male, single and resides at Brgy. Tilambo, Taysan, Batangas. He is known diabetic
for almost 5 years and because of financial constraints, he can’t sustain his
maintenance medications since he doesn’t have a family to provide it. The patient
admitting diagnosis was Hypoglycemia secondary to poor appetite; Anemia secondary
to DM Nephropathy, DM Foot. I’ve handled him on my first hospital exposure and I was
lucky enough that im the one who handled him from the start of his admission until
halfway through of his recovery. My objectives from the very start is to be a helping
hand for the patient towards achieving optimal level of health.

From the start of the admission until the midlle part of the treatment, it was not an
easy journey for the patient as well as the family members who need to thrive for the
condition of the patient. One major problem that make the patient more submerging
health condition is when a positive result of RT-PCR was revealed. He need to transfer
from medical ward to an isolation room for Covid 19 protocols. The patient in that time
needs emotional, mental as well as psychological support since he will be isolated not
only from his family members but also from the other health care team including me
during his treatment. What I do is to serve as a bridge or gap to other resources that I
might not be able to give in tems of financial matter but as long as you connect them to
person who are willing to help and guide them through their recovery is a satisfaction
and a sense of fulfillmet for me as a nurse. Today’s clinical learning environments can
seem overwhelming. Learners, instructors and staff members all face extraordinary
challenges in health care workplaces because of these trying times of pandemic.

In the nursing profession, changes, improvements, and developments are a


common occurrence. New information comes out every day. More efficient ways to
provide care are making their way into nursing practice. Nurses care for people during
some of the most vulnerable and scary times of their lives like what my patient
experince during these trying times of pandemic and usually act as the most regular and
accessible point of contact for patients and families. A great nurse is able to do their job
effectively while also showing compassion, concern, and sympathy for each individual
they serve.

During my almost few days staying on MVM District Hospital for my practicum,
there are valuable added experience which at my age will definitely tell me that nursing
is a continous learning process and we should not be stagnant because from every
moments there are additional knowledge, skills and attitude that we can develop and
can nurture. As a gratitude for the hospital, I've develop a SWOT analysis which might
help the entire organization for the betterment and continous success of it in terms of
quality and safe care for the Bataguenos. First of all are the strengths, these includes
competent nursing staffs from top to bottom, they are all critical thinkers and impose
safe and quality nursing care for all the patients. There are also weakness which are
due to understaffing which is experience worldwide by other organizations, can be
prone by errors or insufficient time staying with the patients to do therapeutic
communications, instead of doing that they attend for some utmost important to do so to
save time. Next is, Opportunity which can be seen that most of the hospital beds has its
siderails to prevent fall for the safety of the patient. And lastly, Threats, because of fear
contacting these virus of pandemic, lenght of staying doing some bedside care to the
patient comes lessens, I know, these are only threats for the patient since we are also
humans protecting ourselves as well as our families at home.

As I ended my reflection for my practicum, what i can say is only these, We


Nurses are the heart and helping hands of the people, we should serve them with
dignity, honesty and integrity, without a foundation of caring, it doesn't matter how good
your nursing skills are.
