HEALTH AND ENTRY REQUIREMENTS

Entry Restrictions
There are no COVID-19 related entry restrictions at this time. However, travellers should hold the relevant travel
documents to enter the country

Entry & Health Requirements

There is no quarantine requirement at this time.

All travellers must present proof of full vaccination showing that they were fully vaccinated at least 14 days before
departure from the first embarkation point. The vaccines must be approved by the National Health Surveillance Agency
(Anvisa) or the World Health Organisation (WHO).

There is no testing requirement for fully vaccinated travellers. All unvaccinated travellers must hold a negative test.
Accepted tests are RT-PCR test or an antigen tests (issued no more than 24 hours before departure). The test must be
issued in English, Portuguese or Spanish.

In-country Experience
Face masks are mandatory in public spaces

Most offices, factories and stores are open with social distancing measures in place

Public transport is operating at full capacity

Public health measures to combat the spread of COVID-19 across Brazil vary between provinces and territories. It is
recommended to check in advance with local state authorities for current restrictions

Transit Restrictions
There are no COVID-19 related entry restrictions at this time. However, travellers should hold the relevant travel
documents to enter the country

Re-Entry Requirements
All travellers, aged 2 years and older, returning to the United States by air must provide a Passenger Disclosure and
Attestation form. This form will be provided to them by their airline at check-in.

All air passengers returning to the United States from China, Hong Kong or Macau must hold a negative test. Accepted
tests are PCR tests or antigen tests issued no more than 2 days before departure from China, Hong Kong or Macau

Other re-entry requirements for travellers returning to the United States vary between states. For up-to-date public health
measures, check the state or local health department of your planned destination. This information can be found here

PASSPORT

You must provide your actual signed passport, including one copy of the personal information page of your passport. Your passport must:

Be valid for the next six months

Have at least two consecutive blank visa pages
Not be frayed, torn, separating, or altered in any other way

PHOTOGRAPH(S)

You must provide a photograph. Upload your photo and we will print it to the exact specifications required for this visa. Follow https://cibtvisas.com/photo-

Alternatively, you may provide one color, passport-type photograph that meets the following strict criteria: be taken within the last three months against an
all-white background and printed on high-quality photo paper; be two inches by two inches; provide the full frontal view of your head with your face
centered in the middle and displaying a neutral (non-smiling) expression. You may not wear eyeglasses or headwear except for religious purposes. Your
photo should not be affixed to your application and the photograph may not display evidence of adhesive tape or staples.

DRIVER'S LICENSE

You must provide a valid notarized copy of your driver's license:

Your notarized copy must display your home address as it appears on your visa application form
The driver's license must be notarized in the state it was issued and display the notary's commission number and expiration date
The driver's license cannot be expired

NOTE: All notarized documents must have an expiration date of the notary commission number.

BRAZIL APPLICATION

The Consulate of Brazil requires a visa application completed online, found at https://formulario-mre.serpro.gov.br/sci/pages/web/pacomPasesWebInicial.jsf.
Click the dropdown in the top right to access the English language page, then click the link for Visa. Your completed application must:

Include your name as it appears in your passport

Include your designated code number which will be issued at the end of the online application process
Include your signature as it appears in your passport
Be submitted to the consulate within 90 days of completion
Be signed inside the LARGE BOX at the top right
When required, upload your passport-style photo. You will not be able to finish the application without uploading a photo. You must also complete the
photo verification process. The photo you upload must be the same photo on the online application. You cannot upload a random photo or your
application will be rejected by consulate.

Applicants under 18 years of age are minors and cannot sign their visa application forms. Both parents must fill out and sign the "Authorization for
Issuance of Brazilian visa for Minor" form and the application form. Their signatures on the authorization form and the visa application form must be
notarized. Minor visas will not be issued without the consent of both parents, except in the following cases:

One of the parents is deceased, in which case an original or notarized copy death certificate is required
One of the parent's is not listed on the birth certificate, or
Court Order or Divorce Decree granting not only full custody but exclusive parental rights to one of the parents or legal guardians. The orginal
or notarized copy of the complete document must be presented along with the other requirements
Note: If the child's parents are Brazilian nationals and the minor has not been issued a Brazilian birth certificate, the Declaration of Non-Citizenship
form must also be completed. The form can be found at http://sistemas.mre.gov.br/kitweb/datafiles/Houston/en-us/file/Declara%C3%A7

In addition, the following items are mandatory for minors:
Copy of both parents' Driver License must be provided

Those taking part in a cruise to Brazil must also present the following: The cruise ship's itinerary with the traveler's name clearly indicated and a

CONSULAR FEES
CIBTvisas fees are per application and are exclusive of Consular fees and third party fees. CIBTvisas submits payment to the appropriate consulate on your
