SGPT (Serum Glutamic-Pyruvic Transaminase)


Sample for SGPT (ALT)
1. This is done on the serum of the patient.
2. The Blood sample can be taken at any time.
3. The fasting sample is not necessary.
4. Analyze the enzyme on the same day because the enzyme activity is lost at room temperature.
5. This enzyme is stable in whole blood for up to 12 to 24 hours; after that time, there is a gradual increase due to release from the RBCs.
6. The sample can be stored at 4 °C for 1 to 3 days (ALT is stable when refrigerated).
1. ALT, when freezed, shows deterioration.

Precautions For SGPT (ALT)


1. Avoid hemolysis because RBC contains increased ALT (SGPT) and AST (SGOT).
2. Drugs that may increase the SGPT value are acetaminophen, aminosalicylic acid, ampicillin, allopurinol, cephalosporin, chlorpropamide,
clofibrate, cloxacillin, codeine, indomethacin, INH, Methyldopa, methotrexate, nalidixic acid, quinidine, phenylbutazone, phenytoin, salicylates,
tetracycline, and propanol.
3. Ideally, perform the test as soon as possible or on the same day because the sample’s storage may not be satisfactory.
4. Previous I/M injection may increase the level.

Indications for SGPT


1. Diagnose liver diseases.
2. Monitor liver diseases after the treatment.
3. It differentiates jaundice from liver disease or hemolytic anemia.

Definition of ALT (SGPT):


1. This enzyme catalyzes the reversible transfer of amino groups between an amino acid, and α-keto acids are called aminotransferase or
transaminases.
2. One of the aminotransferases is Alanine aminotransferase (ALT), or the old name was glutamate-pyruvate transaminase (SGPT) which will
catalyze the reversible transfer of the amino group between an amino acid and α-keto acid where vitamin B6 (pyridoxal phosphate) is the co-
factor.

SGPT (ALT) biochemical reaction

Distribution of SGPT (ALT):


1. ALT is an enzyme found predominantly in the liver.
2. It is distributed in many tissues, with a moderate amount in:
1. Kidneys.
2. A small amount in:
3.  Heart, skeletal muscles.
3. A minimal amount may be found in the pancreas, spleen, lungs, and RBCs.

Variation of the ALT:


1. The day-to-day variation of the ALT is 10% to 30%, and there is diurnal variation.
2. ALT is 45% higher in the afternoon than in the early morning.
3. Until the age of 15, ALT is higher than the AST; after that, AST is higher than the ALT.
4. In a few people, this pattern may reverse after the age of 60.
5. Exercise is the major determinant for AST but less for ALT in men.
1. Strenuous exercise or bodybuilding increases AST 3-fold and a 50% increase in ALT.
2. The effect of the exercise is minimal in the women.

Distribution of SGPT (ALT) in the liver:


1. This enzyme is present in cytosolic forms in the liver. The majority of the SGPT is present in the cytoplasm of liver cells.
2. There is always a low level of this enzyme in the blood.
3. This is a liver-specific enzyme of the transferases.

5. This is also normally found in bile, CSF, and saliva.
6. It is not found in the urine unless there is a kidney lesion.

SGPT distribution in the liver cell

Liver cell injury:

1. Any damage to the liver cells, SGPT enzyme, is released, specifically from liver cell necrosis.
2. First cytosolic SGPT is released, and in the case of necrosis, then mitochondria SGPT is released into circulation.

SGPT (ALT) biochemical reaction and variations in different conditions:


1. ALT catalyzes the reaction, and it is reversible.
2. Pyridoxal-5‘- phosphate  (P-5-P) functions as coenzymes in the amino-transfer reaction.
1. The P-5-P is bound to the apoenzyme and serves as a true prosthetic group.
2. The P-5-P bound to the apoenzyme accepts the amino group from the first substrate, alanine, to form enzyme-bound pyridoxine-5-
phosphate, and the first reaction product is pyruvate.
3. This coenzyme increases the ALT activity by 20% in the normal serum.

4. Hemolysis increases both AST and ALT because of this enzyme activity in the RBCs compared to serum. This effect is more significant for
AST than ALT.

SGPT (ALT) reaction principle

SGPT 1.5 to 8 times (mild or nor elevation) more than the normal level seen in:
1. Early, late and subclinical hepatitis.
2. Chronic hepatitis.
3. Alcoholic hepatitis.
4. Alcoholic fatty liver.
5. Laennec’s cirrhosis.
6. Chemical hepatitis.
7. Passive congestion with centrilobular necrosis.
8. Rye’s syndrome.
9. Cholangitis.
10. Hemochromatosis.
11. Pancreatitis.
12. Granulomatous infections.
13. Neoplasia infiltration.

SGPT >8 to 10 times (3 to 10 times than the normal ) more than the normal value seen in:
1. Mostly in acute viral hepatitis.
2. Chronic active hepatitis.
3. Infectious mononucleosis.

SGPT >30 times (20 or more) more than the normal value is seen in:
1. Drug toxicity like acetaminophen in alcoholics and malnourished people.
2. Viral hepatitis.

SGPT level in various tissue as compared to serum:


Compared to serum (1 unit) in the heart is 450 times; the liver is 2850 times, the kidney is 1200 times, skeletal muscle is 300 times, the pancreas is
130, the spleen is 80 times, the lung is 45, and RBCs 7 times.

AST/ALT ratio:
1. The ALT is the cytoplasmic enzyme in the liver, while AST is both a cytoplasmic and mitochondrial enzyme with a wide distribution in various
other tissues.
1. AST/ALT ratio is typically <1, but infectious hepatitis becomes greater in the liver.
2. AST/ALT ratio is usually greater than 1 is seen in:
1. The patient with alcoholic liver disease (cirrhosis). AST is more sensitive to alcoholic liver disease.
2. Liver congestion.
3. Metastatic tumor of the liver.
3. AST/ALT ratio of less than 1 is seen in:
1. Acute hepatitis.
2. Viral hepatitis.
3. Infectious mononucleosis.
4. The ratio will be less accurate if the AST level is more than 10 times the normal.

Diagnostic application of the SGPT (ALT):


1. The best indicator for liver cell injury is when both SGPT and SGOT are raised.
1. Higher-level are found in hepatocellular disorders than the extrahepatic or intrahepatic obstructive lesions.
2. Levels of both enzymes may reach as high as 100 times. 20 to 50 times, the elevated level is frequently found.
2. Measuring aminotransferases weekly is useful in hepatitis or other hepatic injuries.
3. Because of the accurate measuring techniques, it can detect liver cell injury due to alcohol.
4. It is used as a screening test for diagnosing hepatitis due to hepatotropic viruses, especially the hepatitis C virus in blood donors.
5. 5 to 10 times the elevation of ALT and AST occurs in the liver’s metastatic carcinoma. AST (SGOT) level is higher than the ALT (SGPT). In the early
stages of malignant infiltration of the liver, these enzymes are normal.
6. ALT (SGPT) may be raised after intake of alcohol, delirium tremors, and after the administration of the drugs like opiates, salicylates, and
ampicillin.

6. Cholecystitis.
7. Active cirrhosis.
8. Metastatic tumors of the liver.
9. Obstructive jaundice.
10. Alcoholic cirrhosis.
11. The mild increase is seen in :
1. Myosotis.
2. Pancreatitis.
3. Myocardial infarction.
4. Shock.
5. Infectious mononucleosis.
12. Alcohol-acetaminophen syndrome.
13. A significant increase in level has been in acute liver diseases.

1. A moderately increased level is seen in chronic liver diseases and muscle trauma.
2. A mildly increased level is seen in myositis, Myocardial infarction, and infectious mononucleosis.

Acute Viral hepatitis SGPT (ALT) Pattern:


1. This rises before the appearance of jaundice.
2. The peak level occurs between 7 to 12 days of jaundice.
3. Then there is a gradual decrease in the level.
4. Normal level after 3 to 4 weeks of the onset of jaundice.
5. AST/ALT ratio, which is normally <1, becomes greater > than 1.

2. ALT  (SGPT) is more raised in acute hepatobiliary obstruction than AST (SGOT).
3. ALT (SGPT) is more specific than AST (SGOT) for liver cell injury.
4. The AST/ALT ratio is higher in alcoholic liver diseases.
5. AST (SGOT) is more sensitive to alcoholic liver cell injury.

These levels are in the normal range; I don’t think is serious. If you are very conscious, repeat liver function tests (LFT) after 2 to 3 months.

Your LFT is abnormal; you need further workup like complete LFT with gamma-GT. Ultrasound of abdomen. If you are using alcohol, first quit and recheck
LFT after one month.
If no improvement, then consult a hepatologist or gastroenterologist.

SGPT level fluctuations mean that some minor injury in the liver, the causes may be viral hepatitis C, alcohol intake, some pain killer medications. You
have to rule out these possibilities.

I think your SGOT/SGPT is raised due to alcohol intake. I will suggest stopping alcohol for at least 2 to 3 months till these enzymes are normal. Then you can
take off and on alcohol but not regularly. Otherwise, you will develop cirrhosis of the liver, which ultimately leads to liver failure.

MY DR. JUST ASKED ME TO GET THAT SGPT TEST DONE ALONG WITH THYRIOD. WHEN MY SGPT RESULTS WERE OUT THEY SAID IN FEMALES IT HAS

This is a benign condition and there is no specific treatment. Gilbert’s syndrome is a life-long disorder.
You have to avoid some of the drugs. here are a few of those.
1. Gemfibrozil, for lowering cholesterol.
2. Statins, also used for reducing cholesterol, when taken with gemfibrozil.

Definitely, alcohol will affect SGOt/SGPT and GGT levels. Your cholesterol level is also normal upper limit (borderline case). You have not mentioned the
HDL.
LDL and VLDL are atherogenic and predispose to heart diseases. I will suggest quitting alcohol, and it will damage the liver.

My doc ask me to undergo liver ultrasound and these is what it says:

The liver is normal in size with a span of 13.7cm. Its border is smootu but there is marke increase in the parenchymal echogenicity and the
intrahepatic veins not visible. There is no focal lesion nor calcification
The common bile duct and intrahepatic ducts are not dilated

I drink 3 times a week heavy (270ml in one sitting) from last 10 years. I have been reading on LFT and so did the test.
My SGOT is 38 (0<30) and SGPT is 95 (0<45) rest stuff are all under normal range.

6 months back I have done full body check up and I have found my Sgpt -150 ,sgot-72 then I did test for hepatitis B&C and it also came negative and
then I farther proceed with ultrasound and it came as grade 2 fatty liver and size of the liver was 17.2 .
By that time I was 30 ,hight 164 cm and weight 81 kg.
I controlled my food habits and did regular exercise and now it is showing after 6 months sgpt-72,sgot-48 and other parameters are ok.
How long it will take to come back to normal or any thing to concern and do I need to go for fibro scan

If liver cells are insulted by alcohol or anything else, it will take at least 3 months to regenerate. Also, try to take more fruits. In your case, it is 6 months,
and still, SGPT is raised, indicating liver cell injury. If it remains elevated, then consult a gastroenterologist (or hepatologist).

In your lab tests, SGPT is slightly raised, and uric acid is also. You need to repeat SGPT/SGOT and uric acid after at least 1 to 3 months. If still raised, you will
need medicine for uric acid and a detailed workup for liver function.

My wife is 37 weeks and 2 days pregnant. She had severe itching problem but now it’s resolved. LFT level on 14.02.2022 SGPT 226 ,SGOT 149, ALP 188
,GT 44 all others are in normal range. Fasting bile acid test (4.5 umol/l) with in normal range. no symptom of jiaundice. no other symptoms. HAV IgM
non reactive, HAV IGg reactive, anti HEV IGg and IgM non reactive, HbsAg non reactive and anti Hcv non reactive. PT 10 sec., APTT, 25 sec.,.INR 0.91.
Abdominal ultrasound showed liver has mildly increased echogenicity. After some dietry change and taking hepa merz syrup for 10 days. Repeat LFT
on 24.02.2022, the SGPT 77, SGOT 36 and ALP 176 all other are in normal range. Up till now, according to USG, dilivery is normal. I already consulted
two gynecologists, one,s advice is normal dilivery and other in favor of inducing dilivery at 38 week due risk of baby stillbirth. With these elevated
liver enzyme, can we go for normal dilivery or should induce dilivery at 38 weeks. Suggest some advice.

A gynecologist will give a better answer. Your lab reports say that she has hepatitis A infection and is in recovery. Hepatitis E infection is dangerous in
pregnancy, but that test is negative. I will suggest That you consult the third gynecologist and see what she says.

GGT is not too high. It is an indicator of alcohol intake. I hope he is a non-smoker and non-alcoholic. You can repeat GGT after 1 to 2 months. Let him
reduce his weight. If possible, ask him to take more fruits.

GGT is a very sensitive test. This may be due to medications (drugs). Sometime medication like statin may increase GGT. I think to keep an eye on Liver
function tests.

These liver function tests are in the normal range. Alkaline phosphatase normal values vary from lab to lab. Indirect bilirubin is not significantly high.
Total bilirubin is in the normal range. You can repeat liver function tests after 2 to 3 months.

I really appreciate you helping out everyone. I am male 44 years old and have been doing 48 to 60 hours fasts to lose weight. I have lost around 10 kg,

SGPT/SGOT after one month. As regards being overweight, keep yourself active. You can try intermittent fasting. Control sugary things and carbohydrates.

LDL/HDL RATIO- 1.47 %
TOTAL/HDL RATIO – 3.16
please advice is there any thing to worry?

SGPT is raised, which indicates liver cell injury. This may be due to medications like a pain killers. Triglycerides are also raised. She needs to take a low-fat
diet. Please repeat the liver function test after one month.

hank you for your reply ! Regarding My alcohol usage – I am a social drinker & I used to have alcohol once in a month only , but in last 6 months I used to
ave thrice a week due to work pressure . Now for past 1 month I am not using alcohol. Will it be a reason??

In between of this last 6 months I used to follow my own diet plan ( I Was skipped daily breakfast 4 times in a week , used to have cooked vegtable and liite
urd rice at noon on every day and in Night I used to have rasam rice and curd rice on daily basis ). Will it be a reason ?

y USG Report – ABDOMEN & PELVIS


VER – Liver is normal in size (measures-14cm) & shows normal echotexture , No focal liver lesion .No Intra hepatic biliary radical dilatation seen.
ALL BLADDER – Is adequately distended.No calculus or cholecystitis seen
ANCREAS – Appears normal in size and shows uniform echo texture.The pancreatic duct is normal .No calcifications are seen
PLEEN – Appears normal in size and it shows echo texture. It mesures 11.9 cm in long axis.
GHT KIDNEY – Right Kidney measures – 10.4 * 3.9cm
he shape ,size and contour of right kidney appears normal.
ortico medullary differentiation is within normal. No evidence of pelvicalyceal dilatation.No calculi seen.
EFT KIDNEY – Left Kidney measures -10.4 * 4.8cm
he shape ,size and contour of left kidney appears normal.
ortico medullary differentiation is within normal. No evidence of pelvicalyceal dilatation.No calculi seen.
RINAY BLADDER – Is Normal Contour .No Intra Luminal echoes are seen
ROSTATE – Prostate appears normal ,Measures -2.3*2.5*3cms (Vol-9cc)
GHT ILIAC FOSSA – No Focal fuid collections seen
o Evidence of herina seen in anterior abdominal wall.

nk you are doing good. Once a month social drink will not damage your system, particularly the liver. This is not advisable to skip breakfast. The best would
ake 2 to 4 glasses of water on an empty stomach, then after 30 minutes to take breakfast.

Sir i am 44 yrs MaleHeight = 5.10 ft:inches 88 kg weight & Regular use of Alcohol intake ..since last 12yrs | Take healthy diet + regular execerise (225
heart points/week) as per Google Fit app
Recently done Annual Health Check, and as per ultrasound Liver measures = 12.90cm & both lobes show Grade 2 fatty changes (Grade 2 fatty liver)
however liver parameters show in report as normal :
Total Bilirubin = 0.306 mg/dl
Direct Bilirubin = 0.12
Indirect Bilirubin = 0.19
SGPT/ALT = 20.30
Creatinine = 0.80
All other parameters including cholestrol are Normal. Kidneys Normal, Echo Cardio Normal
Sonography Test Impressions :
Grade 2 fatty liver
Mild Splenomegaly

Sir, Is Grade 2 fatty liver re-versible ?? If yes then, how much time it will take to reverse liver health to normal ?
Is there any indication of liver damage as per above parameters ?

No doubt that liver functions are normal, but due to alcohol use, fatty changes in the liver have started. That will leads to cirrhosis of the liver, which is not
a reversible change. If you stop taking alcohol now, there will definitely be an improvement in liver changes.

Thank you Riaz sir for your guidance..

Till how much time i need to cease/stop alcohol intake in order to completely reverse grade 2 fatty changes ? 4 weeks / 3months / 9 months ? Will not
have alcohol at all during this time.

Can in-take of extract of Moringa Oleifera (Drumstick) + Acacia Arabica (Babool) cause fatty liver / heptacellular changes in Liver ? I take this
combination of extract in warm water daily once for knee joints .

Will go for US (UltraSound) post confirmed period but may not be able to completely avoid alcohol in the future !

I think stopping alcohol is the only answer. If you start again, then again liver will be damaged. If you want alcohol, try having a social drink once a week
or a month.

Hello Dr. Riaz,

I have had hepatitis A for 3 months, my bilirubin has went down to 8.4 mg/dL from its peak at 15.5 mg/dL. However my SGOT went back up from 44 to
59 U/L (previously peaked at 192 U/L), and the same with my SGPT, it went back up from 39 to 83 U/L (previously peaked at 755 U/L). Is it normal to
have fluctuation during the recovery phase?

Hepatitis A is a self-limiting disease. SGOT/SGPT level may fluctuate, and it should become normal when the bilirubin level is normal.

I had posted on this forum as on 22nd Nov 2022 – Alcoholic Fatty Liver (Grade 2) .
As per that report of NOV 2022 :
Total Bilirubin = 0.306 mg/dl
Direct Bilirubin = 0.12
Indirect Bilirubin = 0.19
SGPT/ALT = 20.30
Creatinine = 0.80
All other parameters including cholestrol are Normal. Kidneys Normal, Echo Cardio Normal
Sonography Test Impressions :
Grade 2 fatty liver
Mild Splenomegaly

Yesterday got my LFT + USG Abdomen done : (No change in diet intake + 1 hr daily excercise)
> Completely abstained from any form of alcohol
As per recent report ie 10th Jan 2023 , Grade 2 has changed to Grade 1 now :
Total Protein = 6.77 gm/dl
Albumin,Serum = 4.62 gm/dl
Globulin = 2.15 (bold in report)
A/G Ratio = 2.15
SGOT (AST) = 16.98
SGPT (ALT) = 13.20
Alkaline Phosphatase = 61.27
Total Bilirubin = 0.47
Conjugated Bilirubin(Direct) = 0.23
Un-Conjugated Bilirubin(Indirect) = 0.24
Gamma GT = 24.15 IU/L
In USG report : “Liver is normal in size,outline and shows mild diffuse increased parenchymal echogenicity, suggestive of Grade 1 Fatty Changes. No
focal lesson is seen. IHBRs are not dilated.
Spleen size is normal & outline . (In Nov2022 : Mild Slenogemaly

Your liver functions are normal. The liver fatty change will improve with time. The splenic cyst needs to be further work-up. Please take more fruits which

Ultrasound abdomen shows grade 1 fatty liver changes everything else is normal. She got discharged last sunday and on medication like dollo 650 for
3-4 days after discharge.
