AFFIDAVIT OF NO INCOME AND OPERATION

I, Michelle D.G. Perilla the undersigned is the Chief
Operating Officer (COO) of Serving Individuals For Better
Outcomes In Life (S.I.B.O.L) Therapy Center Inc., a domestic
corporation duly organized and existing by virtue of the laws of the
Republic of the Philippines with office address at 694 Domingo
Santiago St. Sampaloc Manila, DO HEREBY CERTIFY:
1. That from March 2022 to December 2022, the aforesaid
corporation has no operation nor transaction of its business.
2. That on the latest filed Income Tax Return for the year
2022, the business expenses are more than the business income,
hence, no taxable income was declared with the Bureau of Internal
Revenue (BIR).
3. As such, I am executing this affidavit to attest to the
truth of the foregoing and for whatever legal purpose it will serve.
