a. That I shall abide by all the rules and student conducts of the college while undergoing

b. That the college or the barangay shall not be held responsible for any untoward
incident that may happen beyond the scheduled time of the immersion activity.

c. That I understand that while on field immersion, I shall be under the direct supervision

related activities that the policy of No Faculty Supervisor, No Barangay Immersion is in
effect.

d. That the said Field Immersion Program will run for 30 hours.
