1. The Risk-Vulnerability Checklist should be accomplished with an actual field survey of the barangay.
2. One Checklist should be provided for every sitio/purok in the barangay.
3. Members of the barangay council or representatives should be present during the survey and preferably they too fill up a copy of this form.
4. A consolidated checklist result should be prepared for every sitio/purok after the survey if there are multiple surveyors.
5. An accomplished copy of this checklist should be incorporated at the CFPP Output Document.
