content. 

It is Saturday afternoon and I’m just about to close my dental of ce when a 19


year old girl with extreme pain on one of her molars comes in. I notice she has
some degree of intellectual disability so she is accompanied by her mother. Her
mother, having dealt with all sorts of dif culties while raising her up, is
apprehensive and suspicious of everything ad everyone.
Seeing the extreme pain the girl is in, I decide to stay a little longer to take a look
at the teeth that’s causing her pain. The mother seems agitated and desperate,
she is algo a bit rude to the staff and to myself. To avoid a tense environment I
would normally sed the mother to wait in the waiting room, but seeing the girl
need assistance due to her disability I decide to be patient and understanding
and let the mother take a seat next to the dental unit. As usually, I make my clinic
history and listen carefully to the mother.
I examine the girl’s mouth and notice a big ICDAS 5 cavitie on her upper right rst
molar. Due to the deteriorated state of the teeth my diagnosis is irreversible
pulpitis. I then determine that a pulpectomy is necessary as well as antibiotic
treatment. I explain the girl and her mother the disgnosis and the procedures that
need to be performed. I answer all of their questions and have them sign an
informed consent to which they agree. We take diagnostic X-rays and prepare
everything for the pulpectomy.
I place the clinic history, x-rays, informed consent, evolution notes and all other
relevant papers inside the patient’s medical record and keep it in my archive.
I call my trusted endodontist to perform the pulpectomy to make sure that the
procedure is performed in the best way possible with the best quality of care for
my patient, since I know that the periodontist is an expert in what he does and is
well trained. After the procedure is over I put a composite to seal the cavity. We
prescribe the needed medication and send the girl home. Even though there was
no medical emergencies before, during or after the procedure, my dental of ce is
equipped with all the supplies needed in case of an emergency, I have a rst aid
kit and a crash cart.
I keep in touch with the girl’s mother to ensure there were no complications and
that the pain ceased as it should. Everything was reported to be ne.
