I am________________, Teacher I from _________________National High School and I intend

on taking my maternity leave as per advised my OB-GYN, _________________. My child is
currently in the 38th week and 1 day age of gestation and I am expected to deliver him anytime
soon. I am requesting maternity leave from November 16, 2022, to February 28, 2023, a total of
105 days as per mandated in R.A. No. 11210. Moreover, I already have completed the necessary
requirements for the approval of my request. I promise to return and attend to my duty after the
end of my maternity leave.
