    Respected Dean,
    Most humbly to say that my name is Kim C. Lara. I am a student of BS Criminology Section 3-5
    with student number 202011207. I underwent surgery last December 20, 2022. My doctor
    recommended to rest about three to succeeding months. For that, I hereby request a leave of
    absence on January 20, 2023, until the last day of the whole second semester. I do really want to
    attend the class and finish the entire semester, but the thing is I can’t manage to attend the class
    since it is faced to face as per the decision of the school.

    I have also attached medical certificate for your reference.
