II. Background and Rationale

A. Barangay Profile

Insert here the summary of the barangay’s situation based on the

Barangay Profile, RDANA, Preliminary Site Investigation results, PSA
Results, among others. It would also be good to mentions how the
pandemic affected response to other disasters.

B. Problem Statement and Recommended Solution

By briefly discussing the sub-project prioritization process that was

conducted, present: a summary of all identified problems resulting from
the pandemic; the top three (3) priority problems that the community
identified and why these are the priorities; and, the priority problem, the
identified solution that is proposed for funding under the KALAHI-CIDSS
NCDDP AF, and the considerations taken leading to this decision.

References for this section are the PSA Results and the documentation
of the sub-project prioritization activity.

III. Sub-project Description

A. Sub-project Details

Present here the expected sub-project output and other important

features. These shall be based on the technical documents.

Potential impacts of the sub-project and the corresponding mitigating

measures shall also be presented here and these shall be based on the
Environmental and Social Management Plan (ESMP).

B. Sub-project Benefits

Present the: expected benefits and the target direct beneficiaries. These
should be answering the stated prioritized problem and consistent with
the recommended solution (see Section II.B Problem Statement and
Recommended Solution).

State the indirect beneficiaries and the expected benefits.

The necessary information will be based on the PSA results,

documentation of the sub-project prioritization activity, ESSC Results,
results of site validation and field survey, inventory of existing
infrastructure, inventory of available technical resources and market
survey, etc.

C. Sub-project Implementation

Timeline. Present the sub-project duration and the target months for sub-
project implementation. This shall be based on the Construction Schedule
and the project duration reflected in the POW.

Sub-project Proposal

Mode of Implementation. Present here the agreed upon mode of

implementation, whether it will be through Community Force Account
(CFA) or By Contract implementation. This information may be based on
the PSA Results and Inventory of Available Resources.

Construction Method/Implementation Strategy. In relation to the

agreed upon mode of implementation, discuss how the community plans
to proceed with the implementation. The identified challenges and the
mitigating activities shall be included in the discussion. Health protocols to
be observed during implementation shall also be discussed in this section.

Technical Resources, Relevant Health Policies.

Implementation Arrangements. The involvement of the community and

other stakeholders in sub-project implementation shall also be discussed
in this section.

References for this section are the ESMP, PSA Results, RDANA Results,
Inventory of Existing Infrastructure, Inventory of Available Resources,
Market Survey, Technical Documents, etc.

A Gantt Chart showing reflecting the implementation plan shall be

presented. This shall be taken from the Construction Schedule that is part
of the technical documents.

IV. Conclusion

Present the proposal summary (title, location, amount, etc.) and emphasize
the importance why its approval is being requested.
