Dear Nurse

Xxxxx

xxx

I would like to refer a matter for the urgent attention and intervention of the
Nurses Association of Jamaica (NAJ).

The established grievance procedures at my organization, Black River Hospital,

appears to have been compromised resulting in a breach of my employee rights.

In summary, I received a verbal warning in relation to an incident that occurred

on DATE. Subsequently, I received a letter conveying a written warning in relation
to the self-same incident that occurred on DATE. The fairness of the original
verbal warning is yet to be determined as the matter was never arbitrated.

Nonetheless, I complied with and accepted the verbal warning as issued by the
Supervisor Sister ABX as a normal supervisory reprimand.

However, I content that it is unfair and unjust for an employee to suffer two
penalties for the self-same incident: i.e. both a verbal reprimand and in rapid
succession a written warning.

(Include the reference to hospital HR/Grievance manual that speaks to the step in
staff grievances)

Correspondence detailing the incident on DATE is attached herewith.

I await your Attention on the matter and trust that it may be dealt with swiftly.
Regards,
