2. Briefly clarification of focus area (means activities on which trust is engaged).

3. No objection certificate from the owner of the premises.
4. Copy of annual accounts for the period since incorporation.
5. Trust deed.
6. ID Proof of Trustees.
7. Note on activities of the trust since incorporation.
8. Details of donation made during the year.
9. Details of donation received during the year.
10. Details of Banks Accounts including Name of Bank, Branch, Type of Account and
Number of Accounts.
11. Undertaking for Non infringement. (Annexure 2)
12. Telephone Number and E-mail Address of the trust. (Annexure 3)
13. Category of charitable purpose as mentioned in section 2(15) of thee IT Act.
14. Irrevocability Clause (Annexure 4)
15. Provision relating to the beneficiaries are a section of public and not specific
individuals.
16. Provisions relating to dissolution of the trust providing inter alia that in the event
of dissolution of fund/assets will be transferred only to some other trust having
similar objectives. (means dissolution provision in trust deed)
17. Clause in Deed providing the utilization of fund for its objectives only.
