CASE 1

GJ (30 yo)

CHIEF COMPLIANTS :
C/o : intense pruritus on the both hands for 1 week

HISTORY OF PRESENT ILLNESS :


Intense pruritus during all day. Skin lesion appear on the itchy site, erythema and papules followed by
winding threadlike subcutaneous trail of reddish brown inflammation.
Patient feeling discomfort. She applied some calamine cream, but there is no improvement.

PAST MEDICAL HISTORY :

Patient usually doing gardening in the afternoon. She doesn’t wear any gloves when doing gardening.

PHYSICAL EXAMINATION :
BP : 120/80
HR : 88
RR : 17
T : 36.5

Skin lession : erythema and papules at the site of entry followed by winding threadlike subcutaneous
trail of reddish brown inflammation on the both hands.

DIAGNOSE :
Cutaneous Larva Migrans

PLANS :
Explain to pt to wearing gloves when doing gardening because the transmissions occur when skin
directly contact with contaminated soil or sand.

T. Albendazole 400 mg OD 3/7


Cream Albendazole 10% TDS 10 days
T. Loratadin 10 mg OD 1/52
CASE 2 :

LM (45 yo)

CHIEF COMPLIANTS :
C/o itchiness of her vagina started a week ago.
