Introduction
This learning and assessment workbook introduces you to the different methods of
data collection and the guidelines of an effective interview.

At the end of this module, you will be able to:


• Identify three methods of data collection, and give examples of how each is
useful.
• Compare directive and nondirective approaches to interviewing.
• Differentiate closed and open-ended questions, providing examples and
listing advantages and disadvantages of each.

Data Collection
- Is the process of gathering information about a client’s health status. It must
be both systematic and continuous to prevent the omission of significant data
and reflect a client’s changing health status.

OBSERVATION:

- watching a patient's condition but not giving treatment unless symptoms appear or
change.

EXAMINATION:

- Inspection of the body to determine the presence or absence of disease.

- a structured conversation where one participant asks questions, and the other
provides answers. In common parlance, the word "interview" refers to a one-on-one
conversation between an interviewer and an interviewee.

Provide the definition of the following phases and give an example for what a nurse
would do on each phase:

Pre-interaction Phase:

- In this phase, the nurse must become aware of her own personal feelings, fears, and
worries about working with a patient. This self-awareness allows a nurse to accept a
patient's differences without judgment.

Example: To become well known about own feelings, fear, and fantasies; Analyze
professional strengths and weaknesses; Collect information about the patient like
demographic data, occupational data, etc; Prepare a plan based on the data before
meeting.

Introductory Phase:

- It is the first meeting of the nurse with her client (patient). In this phase, the main
goal of nurses is to find out the cause of patients seeking help.

Example: Nurses establish rapport, gain trust and create a familiar situation in which
the patient accepts the nurse; Nurse initiate effective communication; She collects all

the data about clients’ feelings, emotions, strengths, and weaknesses; The nurse
identifies major areas of the problem and plans for the interventions; The nurse
discusses with the client how to solve the problem, they mutually set goals.

Working Phase:

- is where nursing interventions frequently take place. Problems and issues are
identified and plans to address these are put into action. Positive changes may
alternate with resistance and/or lack of change.

Example: Gather more and more data of the patient to explore the stressors; Help
the patient to promote coping mechanisms and develop insight; Let the patient
understand his behavioral change by evaluating himself; Facilitate individual
functioning, If the required nurse can modify it by discussing with the client; The
nurse can evaluate the problems and redefine them.

Termination Phase:

- is the final phase and the period when a patient's goals are assessed and the
relationship comes to an end. The goal of the termination phase is to foster
independence on the part of the patient.

Example: Put reality of separation in front of the patient; Nurse and patient should
mutually explore the feelings, emotions and related behavior; Evaluate the
effectiveness of therapy and achievement of goals; Discuss future plans for meetings
if required.

Communication
- Exchanging information, thoughts, and feelings among people using speech
or other means. The patient conveys their fears and concerns to their nurse to
help them make a correct diagnosis. Thus, the benefits of nurses being able to
communicate effectively are overwhelmingly critical.

- As a Future Nurse, when facing a difficult patient. It’s important to have


compassion and patience. Your patients are often facing difficult situations
that can sometimes affect their attitudes. When you encounter a difficult
patient, take steps to reassure them. Make sure you administer any
medication properly according to schedule and provide extra resources that
might help make the patient more comfortable.

2. How could you have used your knowledge in this/these scenario/s?

- Apply the knowledge and skills that I have learned from school to benefit
others or serve the public good is one exceptionally high impact method to
foster these capabilities across the disciplines.
