Self-Identified Learning Objective # 1

Learning Needs
 What learning needs did I identify through practice reflection and peer input? Identify a
minimum of two (2) to three (3) learning needs.
o The first learning need that I identified through practice reflection and peer input
is to improve on my collection of subjective and objective data.
o The second learning need identified is to improve on reporting SBAR to the next
nurse on shift on Shadowhealth, as well as in a real-life situation.
o The third learning need identified is to improve my interpersonal skills related to
education and empathy with my patients.

Learning Goal # 1

1. My first learning goal is to improve on my collection of subjective and objective data by


the end of the semester. I want to continue to improve every week in Shadowhealth when
documenting my findings. This goal is attainable because I am able to learn from my
previous modules and am able to enhance my documentation skills. This goal is relevant
because it is a big aspect in nursing along with documenting the proper information.

Goal # 1: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activities
with timeframes/timelines.

I will achieve this goal by asking more thought-provoking questions do gather a deeper
understanding of my patients’ condition and past history. The subjective data cannot be
measured but I can continue to ask questions related to the chronic illness. I can also assess and
confirm the subjective data that the patient is reporting by performing tests such as physical and
psychological. The patient’s feelings, perceptions and concerns can also be obtained.

Additionally, I am able to observe the patients’ body language and specific behaviors that the
patient is presenting with. The objective data collected from a patient can be measured, for
example, heart rate, blood pressure, respirations, wound appearance, etc,. I believe I have a
comprehensive grasp on documenting these findings but need to collect more. An example of
this would be the physical data that can be observed by our senses such as the gait test in
Shadowhealth.

These goals can be achieved at the end of each module on Shadowhealth. I am able to compare
the data that I collected with the data the Shadowhealth has and see what I have missed. I am
able to go back into the module and ask deeper questions to achieve that I have met the
requirements of obtaining subjective and objective data and >90%.

Learning Goal # 2
2. The second learning need identified is to improve on reporting SBAR to the next nurse
on shift on Shadowhealth, as well as in a real-life situation. Every other week in
Shadowhealth, we have the opportunity to deliver SBAR to our preceptor. I want to be
able to achieve this goal by the end of the semester and have a thorough SBAR
understanding and report with all important information related to the care of my patient.
This goal is attainable by continuing to practice my SBAR skills and reflect at the end of
the semester from my midterm to my final. This goal is relevant because SBAR is an
important piece of communication amongst health care professionals for information to
be transferred accurately.

Goal # 2: Activities and Timeframes

How am I going to achieve my goal? Identify a minimum of three (3) learning activities
with timeframes/timelines.

I will achieve this goal by collecting more subjective and objective data in order to write an
effective SBAR. This goal relates to my first learning goal by gathering more subjective and
objective data at baseline. Once I have obtained all the information needed, I am able to format
the SBAR.

Situation; clearly and briefly define the situation. This includes the patient name, age, condition
and diagnosis. Background; provide clear background information that relates to the situation,
potential causes for diagnosis. This includes patients’ diagnosis, medications with date and
dosage and laboratory tests. Assessment; what you think the problem is and any changes in
status. Recommendation; what would you like to be done with the patient. This includes and
transfers to another unit, consultations, any further testing, change in treatment that is ordered
and confirm what the treatment plan is.

This goal can be achieved when the SBAR option comes up at the end of the Shadowhealth
module. I want to be able to reflect on my progress from the beginning of the semester and how I
have at the end.

Learning Goal # 3

3. The third learning need identified is to engage in respectful, collaborative, therapeutic


and professional relationships. I want to improve my interpersonal skills related to
education and empathy with my patients. This goal is attainable and relevant to my
practice because as a nurse I need and want to have effective communication skills. This
goal does not have a timeline because it is a continuous cycle that will be used every day.

Goal # 3: Activities and Timeframes


How am I going to achieve my goal? Identify a minimum of three (3) learning activities
with timeframes/timelines.

Some activities that I can use to achieve my goal could be active listening, acknowledging
patients concerns and fears, ask open ended questions, and be aware of my body language. All of
these learning activities demonstrate respect and trust in a relationship.

On Shadowhealth, it can be hard to determine what the patient is feeling or their ability to talk
about their illness. Active listening will allow the patient to continue the conversation in the path
that they would like, as well as maintaining eye contact and open body language.
Acknowledging the patients concerns can demonstrate empathy and sympathy to try and view
yourself in their shoes. Asking open-ended questions can allow the patient to include more
information and emotions regarding their illness, illness experience, care and treatment plan.
Body language can show a lot in a person such as their facial expressions, gestures, “closed-off”
arms, etc,. Maintaining open body language can demonstrate positive listening and active
understanding.

How do my learning goals support my commitment to continuing competency? How does


my learning relate to the competencies of my practice?

My learning goals support my commitment to continuing competency by ongoing self-regulation


and self-reflection on how I am best able to help achieve my patients needs and wants. I am able
to use my knowledge, skills and judgement to practice safely in the hospital setting. I am able to
continue to learn more knowledge and skills with my growth and confidence as a nurse.

The three learning needs that were identified above (improve collecting subjective and objective
data, improve documenting SBAR and improve interpersonal skills including education and
empathy with patients) will over time improve and will be contribute to the quality of the
patient’s outcomes. I want to provide the best possible care that I can, and by continuing
competency I will only achieve more and improve professionalism as a nurse.
