2. Based on what you described above, what are your strengths and areas for
improvement that you bring to this clinical placement? (Areas for improvement
will inform your learning needs).
My strengths in this clinical placement include my communication and
patient-centered care skills. Through my experiences in geriatric care,
palliative care, and emergency care, I learned the importance of being
an effective communicator and providing patient-centered care. I have
also gained experience in understanding different patient populations'
needs and how to meet those needs best. I am also familiar with the
importance of being a team player and working collaboratively with other
healthcare professionals. My areas for improvement that I bring to this
clinical placement include stroke-specific knowledge and skills. Despite
my prior nursing experience, I have limited knowledge and skills in
stroke care. I understand the basics of stroke care, such as recognizing
the signs and symptoms of a stroke and providing general support and
care to individuals who have had a stroke. However, I am still learning
about the latest treatments and interventions for stroke care and how to
implement them in my practice best. Additionally, I want to improve my
communication and patient-centered care skills when caring for stroke
patients and their families. While I am eager to learn more about stroke
care and to enhance my clinical skills further, I am also aware of the
importance of self-reflection and self-care. During my clinical placement,
I want to reflect on my practice and the care I provide to my patients.
Self-reflection is essential for improving my practice and providing the
best possible care to my patients. I also plan to take time for self-care to
stay healthy and balanced throughout my clinical placement. This will
ensure that I provide my patients with the best care possible.
3. What input have you had from your instructors and peers in this program or
clients or healthcare team members from previous nursing roles about your
practice?
As a nurse, I have had much input from my instructors, peers, clients, and
healthcare team members in my previous roles. My instructors have been a
great source of guidance and support, helping me understand nursing
practice's complexities and providing me with the tools necessary to improve
my technique. My peers have provided me with support, feedback, and advice,
helping me to understand the importance of teamwork and shared knowledge
in nursing practice. My clients and patients have been invaluable sources of
feedback, allowing me to gain insight into how my practice can be improved.
Finally, the healthcare team members from my previous nursing roles have
been an invaluable source of knowledge, advice, and support, helping me
better understand the nursing profession. One of the essential inputs I received
while in this program was from my instructors. Through their guidance and
support, I have gained a better understanding of the complexities of nursing
practice and the importance of self-reflection to improve my practice. My
instructors have also been a great source of support for me, offering me
feedback and advice on improving my practice. I have also received input from
my peers, who have shown me how to work better as a team and provided me
with feedback on my practice. The input that I have received from my clients
and patients has been invaluable. Through this input, I have gained a deeper
understanding of the importance of compassion and empathy in nursing
practice. My patients' feedback has been a great source of insight for me,
allowing me to understand better how my practice can be improved.
Furthermore, the healthcare team members from my previous nursing roles
have been a great source of knowledge and insight. Through their experience,
I have gained a better understanding of the challenges and rewards of nursing
practice.

standards of practice and evidence-based approaches to stroke care. This will enable
me to provide the best care for patients in the stroke unit. Connectedness relates to my
learning needs as I need to develop relationships with patients, families, and other
healthcare team members to provide the best care for patients in the stroke unit.
Healing and Health Promotion relates to my learning needs as I need to understand
how to use evidence-based approaches to promote healing, health, and wellness for
patients in the stroke unit. Critical Thinking relates to my learning needs as I need to
develop the ability to think critically and use necessary thinking skills to evaluate patient
conditions and make decisions that are in the patient's best interests. Professionalism
