MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS

This Memorandum of Agreement entered into in Dinalungan, Aurora, Philippines by and

between.

PABLO TECHNICAL SKILLS INSTITUTE INC., a non-stock, non-profit educational

institution, duly recognized and validly existing under Philippines laws, with principal office at
Pob. Zone 1, Dinalungan, Aurora, Philippines herein represents by its School Administrator
and Owner, MR. JOEL DC. PABLO, herein referred to as “PTSI Inc.”.

And

Doctor ______________, a duly licensed Doctor and a resident of Pob. Zone 2, Dinalungan,
Aurora and herein referred to as “DOCTOR”.

WITNESSETH:

WHEREAS, PTSI Inc. needs the medical check-up/medical certificate for the trainees of PTSI
Inc.;

WHEREAS, the DOCTOR will provide the said medical check-up/medical certificate to the
trainees of PTSI Inc.:

NOW THEREFORE, the party agrees as follows:

1. The DOCTOR will provide the following medical check-up/medical certificate with a
charge of one hundred pesos (P100.00) per trainee:
a. Performs preventive nursing care and takes the vital signs of the trainees who will
visit the clinic before the start of the training.
b. Requires to have urinalysis and x-ray
c. Conducts the interpretation of laboratory results.
d. Gives prescription to the trainees with diagnosed medical problem.
2. These nursing services shall be performed by the DOCTOR at the Doctor’s Clinic located
at Pob. Zone II, Dinalungan, Aurora, Philippines.
3. The DOCTOR hereby warrants that all complete accessories, equipment and instruments
are safe, clean and comply with the all the standards and requirements set by the
Department of Health and other pertinent government agencies.
 4. Failure on the part of the DOCTOR to comply with these provisions shall be a ground to
terminate this Agreement, without prejudice to the right of PTSI Inc. to institute the
necessary legal action/s against the DOCTOR to fully protects its interest under the law.
5. The DOCTOR shall hold PTSI Inc. free and harmless from damage, liability, cost, loss or
penalty arising directly imposed or sought to be imposed by or in accordance to this
Agreement.

 MEMORANDUM OF AGREEMENT

This Memorandum of Agreement entered into in Dinalungan, Aurora, Philippines by and

between.

PABLO TECHNICAL SKILLS INSTITUTE INC., a non-stock, non-profit educational

institution, duly recognized and validly existing under Philippines laws, with principal office at
Pob. Zone 1, Dinalungan, Aurora, Philippines herein represents by its School Administrator
and Owner, MR. JOEL DC. PABLO, herein referred to as “PTSI Inc.”.

And

REYNALDO DC. PABLO, a duly registered welding shop, with business address at Santiago
City, Isabela, represented herein as “REY STAINLESS AND WELDING SHOP”.

WITNESSETH:

The parties agreed hereby bind themselves to undertake a Memorandum of Agreement for the
purpose of supporting the School’s On-the-Job Training for trainees under the following terms
and conditions:

1. PTSI INC. shall be responsible for briefing and orientation of the On-the-Job
Training for the trainees who intend to conduct practicum exposure in “REY
STAINLESS AND WELDING SHOP”;
2. PTSI INC. shall issue an official endorsement assuring for the well-being of the On-
the-Job Trainees.
3. REY STAINLESS AND WELDING SHOP may or may not grant allowance, salary
or insurance to the Trainees.
4. REY STAINLESS AND WELDING SHOP may or may not employ the Trainees
upon completion of the On-the-Job Training;
5. The Trainees shall be personally responsible for any and all liabilities arising from
negligence in the performance of his/her duties and functions while under training;
6. There will be no employer-employee relationship between REY STAINLESS AND
WELDING SHOP and the Trainees.
7. The duration of On-the-Job Training shall be equivalent to 240 working hours unless
otherwise agreed by both parties.
 8. This Memorandum of Agreement shall become effective upon signature of both
parties and implementation will begin immediately and shall continue to be valid for
five (5) years.
