At 24 hours before your estimated
time of departure, you are required to
declare any signs and symptoms.
Click the button above to accomplish
the Health Declaration Checklist.

If you have not uploaded yet your


Vaccine Card/Certificate and RT-
PCR/Antigen Test Result, you may
do so when you accomplish the
health declaration checklist.

You must monitor your


registration within 4 hours to 1 hour
before your departure to see if it has a
QR code. Only passengers with QR
code are qualified for the express
lane.

You are now initially registered to the


One Health Pass (OHP) of the Bureau
of Quarantine (BOQ). You are
required to declare any signs and
symptoms you may be experiencing
starting 24 hours prior to your
departure time. You are initially
issued a personal transaction code and
a Barcode. Your registration will be
checked. Once validated, a QR Code
will be issued to replace the Barcode.
Kindly check your registration upon
check-in at the airport.
