Greetings!

Asian College of Technology SHS-Department is planning to push through with the plan of
having face to face classes on August 8, 2022. One of the requirements of the Department of
Education is the Parent’s Consent. May I ask you to fill in the needed information below and
submit the same to your son/daughter’s adviser on or before July 13, 2022 so that we can still
upkeep with DepEd’s deadline
