Pt pain has subsided with 2 tabs of Tylenol #3 (Originally at an 8/10 pain). Pt. appears more comfortable. Pt. came in
with a fracture Rt humeral head # resulting from a fall. Currently wearing a triangular sling. Awaiting orthopedic consult.
Extensive medical history. NKA, FC.

B: Heart attack 15 yrs ago, pulmonary embolism 12 years ago, OA bilateral fingers, CAD, HTN, gastric ulcers 7 yrs ago
r/t overuse of NSAIDs.

A: Neuro assessment WNL. Decreased movement in Rt shoulder. Slight swelling with minimal deformity. Multiple home
medication (Review BPMH). Due to her Hx of gastric ulcers and her current medication ( apixaban ), I believe order of
Toradol should be reconsidered?

R: I think we should D/C the Toradol and consider a different medication. And also review the BPMH

Reflection on the aging population care needs

Age effects the body as a whole. Consider medication absorption and metabolism.
Consider swallowing issues that might occur with age.
With age, patients medical history may be more complicated- might need more medication, leading to more side effects
and poly pharmacy.
Pt. needs to take medication at the same time every day.
To ensure they take their medication:
- Have pharmacy provide medication blister packs to ensure meds are appropriately times and taken.
- Check exploration dates for all medication. do not take meds past due date.
- Set up remainders to take your medication (Use watch or phone alarms).
- Consider PSW/Nurse referral to assist med administration if needed.

For patients with dementia, there might be more resistance to taking medication (belief its not needed, or delusions that
they are poison). Pts are more forgetful and might not be able to let physicians know of any side effects.
Same obligations. ensure correct medication rights when administering medication. might need multiple approaches.

Ageism should not be effect how we treat patients. everyone should have access to healthcare and our elderly should be
treated with dignity. The pandemic showed multiple flaws in our healthcare system that still need to be addressed.

I've had many similar experiences while working on the surgical floor.
Usually insufficient pain management, Having to contact doctor for different meds or different doses.
The person in the Sim appeared more calm than people in real life.
