1. INTRODUCTION
This Project for fabrication of equipment for Project EPC CHF LBCT 2000 TPH PT. AGM,Tbk. And the
equipments will be installed in Lokbuntar - South Borneo.
PT. Meganusa Transmission awarded a Part of Project from PT. Antang gunung Meratus with project
name is “EPC CHF LBCT 2000 TPH PT. AGM,Tbk” and will supply of steel materials, Shop Construction
Drawings, Fabrication and Assembly, Trial Assembly, NDT, Surface Protection, delivery, Construction
Installation and Commissioning.

 Scope
The scope of this section PT. Meganusa Transmission intended method of lifting and installation of
Crusher on site.

PT. Meganusa Transmission will arrange all equipments and other facilities to perform the
lifting/install activities. Table below is shown the list of equipment to be utilized during
stacking/installation.

3. PREPARATION WORK
This preparation is tobe read in conjunction with the rigging Arrangement Drawings. Refer on
Appendix- 1
1. Remove all unnecessary or loose materials and equipment within the vicinity of the lifting
operation.
2. Check the condition of all slings to be utilized to make sure that it is free from surface or cuts
3. All lifting appurtunances shall be load tested. Load test for sling will be conduct and verified
from third party company.

4. Check the cranes and the ground surface condition, additional bearing plate along pathway or on
each of crane position.
5. Make sure all position of cranes according to lifting arrangement drawing

6. Structure is ready for lift up, inform the client for witness prior to lifting and client should have
radio to listen during the execution

4. LIFTING AND INSTALLINGCRUSHER

This operation is to read in conjuction with the lifting and sequences drawings. Refer on Appendix-1
1. Pre- job meeting shall be conducted prior to the lifting operation
2. Barricade the area of lifting operation,positionedboth of mobile cranes (ZOOMLION NK150V)
aslifting crane on the designated location of lifting. Please see detail drawing no. ....................
and make sure radius both of crane same as lifting plan drawing.
3. Install all lifting gear on hook point of hook crane and connect to shackle on crusher pad eyes.
Lift up the structure and check levelness.
4. Lift superitendent to maintain maximum crane load at all crane to avoid over load of the crane.
5. Make sure all activity is finish before release lifting gears. If all activity is finish release all sling.

5. MANAGEMENT OF CHANGE (MOC)

This procedure has been prepared to categories and authorise any intended deviation from the
intended scope of work outlined within this safe system of work.The Lifting director on yard is
responsible for the work being conducted in accordance with the current approved method
statement.
Any deviation from thet approved method is categories as follow:

1. Categories 1- Minor
Lifting director (Person in charge in lifting) on site consider a deviation from the intended scope
of work to be a small adjustment to meet local site condition.They must bring this matter the
attention of the client’ on site-representative. Where the client agrees that the adjusment is minor
and would not introduce any risk to personnel or integrity and safe operation the lifting,

authorisation to procedd my be approved by both parties. Any change must be communicated back
to Lift director, Lifting Engineer,Const Manager and communicated to the work force by means of
toolbox talk, which must also be recorded.The work may then proceed.
Any change from the intended scope of work must comply with lifting procedural, client person in
charge in this lifting. Where there is any doubt as the implication change may have on the safety
and/or integrity the crane or any conflict from the client’s representative on site, for whatever
reason, this change will become a category 2-Mayor.

2. Category 2 – (Major) Deviation


Category 2 deviations are those that the Lift director or the client’s nominated representative
considers to be significants or deviations from category 1 that become category 2 as identified. Any
proposed deviations/ changes to the intended scope of work under this category shall be
comunicated to the Lifting Engineer/Construction Manager who will revise the method statement
and liaise with the client representatif to agree any changes for approval prior to lifting. Where
necessary, engineering deviations will be approved by the client . On approval, any changes in the
method statement scope of work affecting the safe system of work will be communicated to the
work force by means of recorded of toolbox talk. All deviations and/or changes will be recorded and
authorised within the record box provided within this method statement.
A record of authorised changes during scope of work is available within the tempalte method of
statement devised for the erection and/or operation of lifting activity. Example provided below,
