Dear {{Name}}:
Your activities will consist of {{Describe Program Activities}}. Continued participation in The
{{University Location}}’s exchange visitor program is contingent upon satisfactory performance as
determined by me and maintenance of non-immigrant status. All inquiries you may have concerning
maintenance of {{status}} and other immigration related matters should be directed to {{Immigration
information service desk}} {{Immigration Address}} {{ Immigration email}}, {{ Immigration phone}}.
Funding
{{Funding Option}} The {{University Name}} shall provide a stipend of ${{Dollar Amount}} per
{{Month/Year}}. The University will also provide medical insurance starting on the 43rd day of your
employment. You will be responsible for purchasing health insurance from UB’s Student Medical
Insurance Office for the 42-day waiting period before your employer-sponsored health insurance
takes effect. The {{University Name}} will also provide you with {{office}}/{{lab}} space {{which may be
shared}}, the use of a computer and access to library facilities.
{{Funding Option}}
It is my understanding that you will provide your own funding to cover all the expenses of your stay
at the {{University Name}}, including health insurance for yourself and your accompanying
dependents, if any. The {{University Name}} will provide you with {{office}}/{{lab}} space {{which may
be shared}}, the use of a computer and library facilities.
Housing
Unless our department explicitly states in writing that it will plan for you, it will be your responsibility
to find housing for yourself {{and your family, if applicable}} for the duration of your stay at the
University. Helpful tips for finding off-campus housing can be found at Information on costs of
housing and transportation can be found at {{website}}.

Health Insurance
The {{Organization}} to have medical insurance coverage during their stay in the Exchange Visitors.
The {{Organization}} also requires that dependents have medical insurance that meets
{{Organization}} regulatory requirements. Further information about insurance for {{Type of scholar}}
scholars is available at {{website}}.
To affirm that you and your dependents will have the necessary health insurance coverage, we
require you to sign and submit the Medical Insurance Attestation form to us. After you have done so,
we will send you a {{Form name}} so you can apply for a {{Type of Visa}}. The Medical Insurance
Attestation Form is available at {{website}}.
Although you may wish to purchase health insurance in your home country, you should be aware
that your home country policy can only substitute for the {{Policy type}} policy if the health insurance
coverage is comparable as determined by {{University}}’s office of Medical Insurance. If not, you will
also have to purchase the {{Policy type}} policy. Similarly, any health insurance policy purchased for J-
2 dependents must meet the {{Organization}} requirements or the dependents will also need to
purchase the {{Policy type}} policy. Therefore, we do not recommend purchasing health insurance in
your home country to cover you during your stay in the {{Location}}.
NOTE: Although Immigration Services will issue your Form DS-2019 and will serve as your sponsor at
{{Name of Department}}, it does not evaluate insurance policies; only the Student Medical Insurance
office can do this. You can have your policy evaluated by sending your policy and a completed waiver
form to {{University Name}}, Student Medical Insurance, {{Address}} or contacting the Student
Medical Insurance office at {{email}} . Waiver forms can be found at {{website}}.
Please make time now to familiarize yourself with {{University Name}}’s Exchange Visitor Program as
described at {{website}}.
We are delighted that you will come to our University as a {{Position Title or “Visiting Scholar”}} and
look forward to meeting you.
Sincerely,
{{Name}} {{Title}}
