SPECIFIC INSTRUCTIONS TO PATIENTS FOR CAUTERY PROCEDURE:

1. You have to be asymptomatic at least 14 days prior to your scheduled procedure with NO
KNOWN exposure to a COVID-positive patient.
a. If you develop fever, cough or other symptoms until the day of your schedule, please
inform us so your schedule can be properly cancelled and given to another patient.

2. PRIOR TO THE PROCEDURE:

a. My secretary will get basic info from you prior to your schedule. This includes: NAME,
AGE, ADDRESS, CONTACT NUMBER and EMAIL.

b. I will send you an informed consent via viber message. Please answer “yes” or “I agree.”

c. After agreeing to the informed consent, please send 1-2 pictures of the area with warts for
documentation.

d. Please know the post-procedure instructions, too.

i. DON’T wet the area for 3-4 days. Clean the area with betadine daily until the
wound closes.
ii. DON’T EAT allergenic food for 2 DAYS. This includes: seafood, oranges and
mangoes, chocolates and nuts, chicken and eggs, alcoholic drinks, artificial food.
iii. IF NEEDED, YOU WILL BE GIVEN ORAL MEDICINES to take. Prescription
will be sent thru viber a day before.
iv. Notify your doctor for any untoward incident.

e. We strongly encourage cashless transactions nowadays. Please pay the cost of the
procedure online either by bank transfer (BPI 1749224165 / Security Bank
0000017627977). I will let you know the cost of the procedure and the PPE.

3. ON THE DAY OF THE PROCEDURE:

a. Go to the Garden heights at least 20 minutes prior to your schedule. You may park your
car at BASEMENT 1, NUMBER 17. Park behind my car. That slot is reserved for you.
Just inform the guard on duty at the entrance of the parking lot that you are my patient on
schedule.

b. PLEASE WEAR MASK AND FACE SHIELD AT ALL TIMES.

c. You have to go through the lobby checkpoint 15 minutes prior to your schedule. Your
name has been given to the guard already. We expect you to be at the clinic at least 10
minutes prior to your schedule.

d. Please bring a small bag only for your immediate needs. Your bag will be placed in a
clean plastic bag upon entry in the clinic.
e. You will also be given a set of BOOTIES to wear in the clinic. You may take them home
with you.

f. You will be asked to clean your hands with alcohol for 20 seconds, and wear clean
gloves from the clinic. You are not allowed to use the clinic washroom as this will be
used for doffing used PPEs and will not be fit for use by patients for safety purposes.

g. NO COMPANIONS are allowed inside the clinic unless necessary (e.g. very elderly,
pediatric patients). We will only accommodate one person at a time. If you are coming
with another patient, we will allow one more person inside the clinic but maintain social
distancing while waiting for his procedure.

h. In the clinic, after personal verification, you will be ushered inside the procedure room
for your treatment.

i. I hope you understand that during these COVID TIMES, we have to limit your time in
the clinic. After the procedure, you may leave the clinic since everything has been settled
already. We disinfect the clinic every after patient for the next patient.

j. Just message me anytime via viber for questions/ clarifications.
