    This is to explain late forwarding of medication card during MICU/CCU Ward Rotation
    last February 21, 2023 during our Week 5 RLE Rotation, under the clinical supervision of Ms.
    Maida Faith D. Basilio.

    I sincerely apologize for late forwarding of medication because I had been busy on the
    besides doing cold compress, assisting in resinsertion of IV catheter and doing close monitoring
    of the patients vital signs because he is having hyperthermia, rising systolic, declining diastolic
    and irregular respirations.
