This document needs to be fully completed and a copy of the ID/Passport/NIE of the Authorizing
Person must be shown. It will be returned to the Authorized Person once the data are checked.
For the delivery of the shipment, the Authorized Person must prove its identity by means of its
ID/Passport/NIE.

Personal data provided will be processed by DHL Parcel Iberia, S.L.U., Paseo Mikeletegi 65
20009 San Sebastián, to prove the delivery of the shipment and on the basis of the consent
granted by signing this document. Data will be kept as long as they are necessary for the
purpose for which they have been collected and during the applicable legal provisions.
Transfers of data are not foreseen, except legal obligation. To exercise the rights of access,
rectification, deletion, opposition, limitation or portability, you can contact
lopd.spain@dhl.com. You can file a claim to the AEPD at www.aepd.es
