TRAFFIC ACCIDENT REPORT

I. NATURE OF CASE:
Reckless Imprudence Resulting to Multiple Homicide and Multiple Injuries with damage
to Property.
II. TIME/DATE/PLACE COMMITTED.
At about 6:00 in the Morning of 11 August 2021 at Barangay Santiago Concepcion.
III. VEHICLE AND DRIVER INVOLVED
Vehicle 1- (Bus) Philippine Rabbit Passenger bus with body number 1024 with the
Plate number ABC-123, Driven by JUNE MALLARI y Cruz.
Vehicle 2- ISUZU dump truck loaded with gravel and sand with the plate number
ADD 1223, Driven by ERNESTO MENDOZA y Gomez.
Vehicle 3- (BUS) Victory Liner passenger bus with the plate number ASS-1234 with
the body number 3345 driven by MAMERTO DELA CRUZ y Garcia.
Vehicle 4- TOYOTA Corolla with the plate number AAA-1234 Driven by
FERNANDO ERESE y Javier.
IV. VICTIMS
1. ARTHUR TUGATE y Diaz, 32 years old Passenger of Philippine Rabbit.
A. Currently The manager of cigarettes company in Pampanga and
Quezon, City.
B. Arthur Tugate, Currently traveling going to work.
C. This victim injured in Legs and Arms due to strong impact his left arm
dislocated.
D. Confined in Concepcion District Hospital.
E. Single.

2. JEJOMAR BINAY y Ramos, 39 Years old Conductor of Philippine Rabbit.

A. 4 Years working in Philippine Rabbit as a conductor.
 B. JEJOMAR, Injured in legs and neck due to strong impact his neck
didn’t move.
C. Travelling to Manila.
D. Confined at Concepcion District Hospital.
E. Married with 2 children.

3. ALBERT MARTINES y. Gao, 48 year old and 9 years Conductor of Philippine

Rabbit.
A. Currently working in Philippine Rabbit in almost 9 years.
B. ALBERT, Multiple Injuries in different part of his body.
C. Delivered in the Private hospital due to his critical injury.
D. Married with 3 children.

4. MARIO DE VERA y Gando, 22 years old Passenger of Victory Liner bus.

A. Student of Tarlac State University as a Psychology student.
B. Currently living at Cubao, Manila
C. MARIO, Unconscious due to strong impact, But in initial investigation
this victim injured in arms and legs.
D. Delivered in the Private hospital due to his injuries.
E. Single

5. ALYSA VALDEZ y David, 38 years old 6 months working as a Conductor of

Victory Liner bus.
A. Currently working in Victory liner in almost 6 months.
B. Living in Mabalacat, Pampanga
C. ALYSA, Escape unhurt.
D. Single

6. SILVIA DELA TORRE y Lalic, 31 years old 6 months working as a conductor

of Victory Liner.
A. Currently Working in Victory liner in almost 6months.
B. Currently Living in Bamban, Tarlac.
C. SILVIA, lost of Consciousness due to strong impact with multiple
injury.
D. This victim delivered in the nearest hospital due to Critical injury.
E. Married with 5 children.

7. And the Injured Husband and Wife riding in Toyota Mr. GILBERT BAIT y See
and Mrs. DELIA DELA CRUZ y Javier.
A. MR. GILBERT 31 Years old, working as a teacher and MRS. DELIA
working as a Government employee Currently living in Concepcion
Fiesta Community.
B. Travelling to SCTEX Going to manila to business meeting.
 C. Mr. GILBERT AND Mrs. DELIA Escaped Unhurt but delivered at the
Provincial Private hospital for further check up.

8. Mr JUNE MALLARI y Cruz Driving the Philippine Rabbit 47 years old.

A. Currently working in Philippine Rabbit as a Driver in almost 10 years.
B. Holding a Professional License.
C. Currently living in Avenida, Manila
D. Travelling to SCTEX Going to Tarlac.
E. Married with 3 Children.
F. Mr. JUNE, In critical condition DOA. (Under autopsy)

9. MAMERTO DE LA CRUZ y Garcia Driver of Victory Liner Bus 47 years old.

A. Currently Working in Victory Liner in almost 13 years.
B. Holding a Professional License
C. Living at Mabalacat, Pampanga
D. Travelling to SCTEX going to Tarlac.
E. Married with 7 Children.
F. Mr. MAMERTO, DOA

10. ERNESTO MENDOZA Y Gomez Dump truck Driver. (No any personal
identification)
A. Working at Dunkin donut Security Guard. (Sighted in his Compamy
uniform)
B. Unidentified License
C. Travelling to SCTEX loaded with gravel.
D. Single.
E. ERNESTO is in critical condition delivered in Provincial Hospital due to
Critical condition.

V. FACT OF THE CASE:

Initial investigation shows that while Mr JUNE MALLARI y Cruz was driving the
Philippine Rabbit bus liner with body number 1024 and plate number ABC-123, coming from
Manila to Tarlac City and while travelling the SCTEX road on minimum speed and upon reaching
kilometer post number 117 in the vicinity of Barangay Santiago Concepcion, Tarlac, he noticed
a thick smoke from a grass fire on the left side of the road and at that instant , he reduced his
vehicle speed.
unfortunately , the following ISUZU dump truck loaded with gravel and sand with plate
number ADD-1223 then being driven by ERNESTO MENDOZA Y Gomez did the same but
unfortunately his vehicle's brake loose and as consequence , hit and bumped the rear side of the
bus causing the bus to pushed forward to the right ,rammed to the center steel bar and pushed
further causing the said bus to enter on the opposite lane.
 ERNESTO MENDOZA Y Gomez, Sighted on the previous camera swaying and Improper
Overtaking.
Due to the incident, the speeding and incoming Victory liner bus with plate number ASS-
1234 with body number 3345 then being driven by MAMERTO DE LA CRUZ y Garcia collided
to the Rabbit Bus and due to the strong impact, the two bus liner turned turtle and occupied the
whole two way lane road.
Immediately after the impact , a speeding TOYOTA corolla car with plate number AAA-
1234 then being driven by FERNANDO ERESE y Javier which tried to avoid the two buses
collided to the victory liner bus causing a accident along the SCTEX road.
Accident resulted to multiple injuries and death to the driver of two buses and the driver of
the car which were pin down in their respective driven vehicles and also resulted to the injuries of
Rabbit bus liner passengers namely ARTHUR TUGATE y Diaz , (Conductor) , JEJOMAR
BINAY y Ramos, ALBERT MARTINEZ y Gao , injuries to Victory liner bus passenger were
MARIO DE VERA y Gando, (conductor) , ALYSA VALDEZ y David, SILVIA DE LA TORRE
y Lalic . Injuries to Car were Gilbert Bait y See and his wife Delia De la Cruz y Javier, and the
dump truck driver and his occupants escaped unhurt.
All fatality were brought at Funeral this town for autopsy while injured persons were
brought at Concepcion District hospital for treatment and the other bring to the private hospital
due to critical condition. Other passengers of both bus sustained injuries.
VI. ACTION TAKEN:

Ocular Investigation conducted and photograph were taken, Suspects driver and Vehicles
were at Police Station for further Investigation.

VII. STATUS OF THE CASE:

A Case of RIR to Multiple Homicide and Multiple Physical Injuries were being prepared against
the suspect driver.
