expense reimbursements, bonuses, and other employment-related or government benefit payments (“Payments”) from the Payor by electronic funds transfer to my Titanium+
Prepaid Mastercard® (“Card”) issued by Peoples Trust Company. I authorize the Payor to make Payments to my Titanium+ Prepaid Mastercard® by initiating electronic entries
(and, if necessary, debit entries and adjustments for any credit entries in error) to my Titanium+ Prepaid Mastercard® account. I understand that if I have arranged to have direct
loads made to my account at least once every 60 days from the same Payor, I can visit mytitanium.ca or call 1-866-914-5863 to find out whether or not the load has been made.
I understand that: (i) I may cancel this authorization at any time by sending a written notice to the Payor at least 5 business days in advance of any payment; (ii) Payor may
terminate this method of payment, with or without cause, at any time; (iii) Payments made to my Titanium+ Prepaid Mastercard® will be subject to the terms of my Titanium+
Prepaid Mastercard® Cardholder Agreement within Peoples Trust Company; (iv) I may continue to receive Payments by cheque or (if applicable) direct load to a Peoples Trust
Company account until this authorization is processed and my Payments are added to my Titanium+ Prepaid Mastercard®; and (v) funds from electronic DirectLoad will
generally be available on the day Peoples Trust Company receives the load. I also understand that, in case of transmission error or load irregularity, my ability to withdraw funds
may be delayed beyond the day Peoples Trust Company receives the load, in which case the funds will generally be available for withdrawal within five business days after the
load. Your Prepaid Card Account and associated DirectLoad account number cannot be used for preauthorized direct debits from merchants or from utility or Internet service
providers. This card is issued by Peoples Trust Company under licence from Mastercard International Incorporated.
