CHECKLIST OF REQUIREMENTS
MINERAL PROCESSING LICENSE APPLICATION

MANDATORY REQUIREMENTS
The applicant shall submit at least five (5) sets of the following
mandatory requirements:
1. Duly accomplished Application Form (MGB Form No. 11-2)
2. Filing Fee of Fifty Thousand Pesos (PhP 50,000.00)
3. For Corporation/ Association/ partnership/ cooperative: Duly
certified Certificate of Registration issued by the SEC or authorized
government agency concerned.
3.1 Registration Fee: PhP 1,020.00
4. For Corporation/ Association/ partnership/ cooperative: Duly
Certified Articles of Incorporation/ Partnership/ Association and By-
Laws, with a minimum authorized capital of PhP10 Million;
4.1 Registration Fee: PhP 1,020.00
5. Sworn Board Resolution signed and executed by the members
of the board authorizing the President or Chairman of the company/
applicant to execute documents in behalf of the corporation;
5.1 Registration Fee: PhP 220.00 per document
6. Location Map / Sketch Plan (1:50,000 NAMRIA topographic
map) of the area of the proposed processing plant.
7. Feasibility Study including work programs, plant site, mill and
plant lay-out/design, details of technology to be employed in the
proposed operation, anti-pollution devices/measures as well as the
plant capacity, duly prepared and signed by a licensed Metallurgical
Engineer: Provided that the applicant with existing small-scale
mineral processing plant may submit in lieu of the Feasibility Study,
a Project Description and a detailed financial statement for its
operations incorporating the social and environmental expenditures,
taxes and fees paid (MGB Form 5-3A)
8. Community Development & Management Program (CDMP)
9. Potential Environment Impact Management Plan (PEIMP)
10. CEMCRR or Certificate of Exemption, in lieu of a CEMCRR, if
the applicant has neither past nor present mineral resources use or
mining-related venture(s);
11. Proof of Technical Competence in the form of:
11.1 Bio-data and track records in mining operations/
environmental management
11.2 Sworn statement or employment contract/ proof of
availed service of the technical person/s who will
undertake the implementation of the Work Program.
12. Proof of financial capability
12.1 Latest Audited Financial Statements
12.2 Annual Report for the preceding year
12.3Credit Lines/ Bank Guarantees/ Deposits
13. Certification from the EMB and other government agencies
concerned on the use of chemicals in compliance with the
existing laws, rules and regulations, if applicable;
14. Supply Contract/ Agreement with mining right holders, if
applicable
