 Two passport size photographs in school uniform with white background.
 Photocopy of ‘B’ Form or Family Certificate
 Photocopy of previous result of As Level
 Photocopy of smart card or CNIC or Passport of the student

FEE DETAILS:

The school fee must be paid to qualify for CAIE May/June 2023 Registration.
i. Date of Submission of CAIE Registration Form will be ______________with the paid
copyof school fee of (Semester 2) 2022.
ii. Cambridge fee Voucher will only be issued if the school fee is paid till (Semester 2) 2022.
iii. Payment deadline for Cambridge Fee voucher is _________________________.
iv. If you select any wrong subject code or subject title then school will not be responsible
for anypenalty. If there is any possibility of amendment then all the charges will be paid by
students.
v. Provisional statement of entries will be signed from the students on and if any student has
not signed provisional statement of entry then school will not be responsible.

vi. C A I E Fee bill would be issued after the submission of this form.

vii. No form will be entertained without the student / Parent/coordinator signature.

viii. In the case of misplacing this form, Reissuing fee is Rs: 200 for this form.
