OVERVIEW
The Minto-Mapleton Family Health Team is excited to share its
2023/24 Quality Improvement Plan that focuses on three main
areas: preventative screening, patient use of opioid medications
and patient involvement in decision-making. The biggest room for
improvement for our Team rests with our preventative care
screening, specifically colorectal screening, breast screening and
cervical screening. As a result of COVID, many patients have fallen
behind with their preventative screening and our goal for the up
and coming year is to get back to pre-COVID screening rates. We
will be using our own data to help monitor our progress as the
reports that we receive externally are often dated and not
reflecting the current situation. We will also be expanding our
digital communications in 2023/24 which will provide a greater
opportunity to inform our patients and remind them of the
importance of getting their preventative screening updated. We will
expand our electronic patient survey which allows us to regularly
receive patient feedback on a timely basis. Quality improvement
remains a priority for the Minto-Mapleton FHT and our Quality
Committee has agreed to meet quarterly to monitor our progress
and tweak our initiatives along the way, as needed. We look
forward to reporting on our progress as the year unfolds.
