Please be informed that starting this January 2020 all Loan Contracts must be accurately and
completely filled up and signed as the loan contract requires.
Please observe and comply the following as per required in loan application and contracts:
 Fill up the Loan Application Form completely and accurately with signature and complete
name of the spouse and the member and do not forget to write the contact number of the
borrower.
 Indicate the existing loan amount, balance, status of the previous loan and updated CBU of
the borrower found in Member’s Account –Verification Sheet Form.
 Indicate the “Term of Loan” and “Mode of Payment” and explain precisely the “Purpose of
Loan” indicated in Application for Loan Form and must be signed by the borrower, spouse
and Co-maker with complete printed name same as in Loan Note Form.
 Attach the Xerox copy of valid ID of the Borrower, spouse and the Co-maker. Verify that
their signature is the same signature in their ID.
 Use the following to indicate the type of loan:
 Livelihood/Farming
(Indicate whether “Palay Production” / Fruits & Vegetable Production / Fishery/
Livestock / poultry etc… as required by the LBP)
 Personal Loan (all appliances loan)
 Business Loan
 Emergency Loan
*Note: Do not use CASH LOAN as type of loan in any LOAN forms/Contract

All incomplete contracts will be return to the branches as what have been discussed and agreed upon
the Meeting held last November 13, 2019.
