JP: For our 3rd Eviction night, housemates Macie, Jek, Paul was evicted because of a mistake
they did on a specific task inside big brothers house specifically the failure mode and effect
analysis. It eventually made them realize that the damage has been done. People are very
quick to judge them, call them names, and crucify them on social media that these engineers
doesn’t know the gravity of words spoken. They had learned their lessons and this will help
them grow and mature in life.

– PBB Concept – There was a task na bingay si Big Brother sa mga housemateS at yun ang
mag examine ng video – ( pilot got stuck outside window of plane ) nagawa naman nila yung
analysis or FMEA which is yung pinakagoal ni kuya sa task pero di naging sapat yung mga
naging answers nung tatlong na evict --- napalabas sa bahay ni kuya – 3rd eviction night –

Isa isag tatawagin yung tatlo una si macie sunod brazan huli si jek – mangangamusta si toni sa
housemate 1 tas magtatanong na kung may pagkakataon pa para mas ma explain nila yung
sarili nila about sa task ano pa yung gusto nila sabihin na hindi nila nasabi sa loob ng bahay ni

What is Failure Mode Analysis?

Failure Mode and Effects Analysis (FMEA) is a structured way to identify and address

potential problems, or failures and their resulting effects on the system or process before an
adverse event occurs. In comparison, root cause analysis (RCA) is a structured way to address
problems after they occur.

Purpose of Failure Mode Analysis

FMEA studies provide the means to evaluate risk levels thereby establishing a mechanism to
prioritize risks. They lead us through a step-by-step approach of analyzing the design or
process, understanding how it is supposed to function, identifying how it can fail to function as
intended, determining the consequences of the potential failures, assessing the impact of the
consequences, the frequency of occurrence and the ability to detect the failure.

Pilot sucked out of plane: Miracle of Captain Tim Lancaster

On 10 June 1990 an improperly installed panel of the windscreen failed, blowing the plane's
captain, Tim Lancaster, halfway out of the aircraft, with his body firmly pressed against the
window frame.
