Hours of Work Employee/Employer Agreement

The inventory stocktaking and data collection business is by definition cyclical, with busy and
slow periods spread throughout the year. In order to meet the needs of our customers, WIS International
may occasionally need employees to work more hours than the provincial standards set by the Ontario
Ministry of Labour in the Employment Standards Act.

Attached you will find the Information for Employees About Hours of Work and Overtime Pay
document provided by the Ontario Ministry of Labour.

WIS International is submitting an application to the Ministry to have their approval to work more
than 48 hours per week, up to 60 hours per week.

In order to make this application we are required to provide our employees with a copy of the
attached information sheet and give you the choice to work more than 8 hours per day and more
than 48 hours per week.

The requirements for rest periods and breaks are specifically outlined in this information sheet.

WIS International does not average overtime, all hours of work over 44 hours in a work week are
paid at the required overtime rate of pay.

I understand that this agreement does not guarantee a specific number of hours of work, but that
the hours of work in each work week vary with our customer’s schedules.

I understand that I have the right to revoke this agreement by providing 2 weeks written notice to
WIS International.

By signing this agreement I am confirming that I have read the attached information guide and
agree to its content and agree to work more than 48 hours per week, up to 60 hours per week when the
schedule requires it.

to work more than one shift with less than 8 hours between shifts.

document provided by the Ontario Ministry of Labour.

In accordance with the act, we are required to have written agreements with our employees to
attain their approval to work more than one shift in a day with less than 8 hours between shifts.

The requirements for rest periods and breaks are specifically outlined in the attached information
sheet.

I understand that this agreement does not guarantee a specific number of hours of work, but that

I understand that I have the right to revoke this agreement by providing 2 weeks written notice to
WIS International.

By signing this agreement, I am confirming that I have read the attached information guide and
