AFFIDAVIT OF NO OTHER DONATION

I, ELSIE AVERGONZADO, Corporate Secretary of SVCPH, INC., with address at 10F i1 Building,
Cebu IT Park, Apas, Cebu City, after being duly sworn in accordance with the law, do hereby depose and
state that:
1. I am the duly appointed Corporate Secretary of SVCPH, INC. (the “Corporation”), a corporation
duly organized and existing under and by virtue of the laws of the Republic of the Philippines,
with principal office at 10F i1 Building, Cebu IT Park, Apas, Lahug, Cebu City.

2. At the special meeting of the Board of Directors of the Corporation held on Dec. 1, 2021, during
which quorum was present and acting throughout, the following resolution was approved:

“RESOLVED, that in order to aid those affected with the Coronavirus Disease 19 or
COVID-19, a total sum of ONE MILLION PESOS (P1,000,000.00) will be given and
transferred to affected families by way of donation.

3. This is the only donation made by the Corporation for the year 2021.

4. This affidavit is being executed to attest to the truth of the foregoing facts and for all legal intent
and purpose it may serve.
