Below is a consent form for your son/daughter to participate in the athletics event in Sportsfest
2023. This consent form allows your child to join the try-out on their respective sport for the said
event. The undersigned below, parents/guardians, understood that the school is not liable for
any incident that may happen during the try-out / training. Also, it is understood that training may
be held away from the school.

Their try-out will be held on February 17, 2023 in San Roque, San Rafael Bulacan (San Roque,
Covered Court). try-out venue is subjected to change depending on its AVAILABILITY.
