Two (2) Form 1 forms as contained in the First Schedule of the Citizenship Regulations made pursuant to the Citizenship Act, Chap. 1:50 obtainable at the Ministry of National Security Port-of Spain. These forms must be completed by the applicant and signed by the applicant in the presence of a Commissioner of Affidavits. 2 A non-refundable fee of $50.00 payable at the Ministry of National Security. The Total fee is $320.00. The balance shall be payable when the application is approved, 3. Birth Certificate. If correct name, etc., does not appear on the Birth Certificate then an ‘Affidavit sworn to by a person who knows of birth, namely parents, elder siblings or older relatives must be submitted, or Deed Poll- Original and One (1) Photocopy. 4. Three (3) identical passport size photographs. 5: Parents Birth Certificate - Original and Photocopy. 6. Grand-parent Birth Certificate - Original and Photocopy. 7. Marriage Certificate of parents (if applicable) - Original and Photocopy. 8. Marriage Certificate of grand-parent (if applicable) - Original and Photocopy- 9. Passport of Child - Original and Photocopy. 10. Passport of Parent - Original and Photocopy. 11. Passport of Grandparent - Original and Photocopy. 12, Certificate of Immigration Status (C’I.S,) of child - Original and Photocopy. 13. One (1) self-addressed stamped envelope. (9x4)
